Arizona Medical License Renewal: Fees, CME, and Deadlines

Arizona medical license renewal happens every two years: allopathic (MD) physicians renew on or before their birthday, and osteopathic (DO) physicians renew on or before January 1.¹² Miss the deadline and a penalty fee kicks in; wait four months and the license expires automatically. MD renewals go through the Arizona Medical Board (AMB), and DO renewals through the Arizona Board of Osteopathic Examiners in Medicine and Surgery (AZDO). The two boards charge different fees and follow slightly different procedures.

Renewal Deadlines and Grace Periods

For allopathic physicians, the deadline is your birthday in the renewal year. You have a 30-day window after that date to renew at the standard fee with no penalty. From day 31 through four months past your birthday, you can still renew, but a late penalty applies. Four months past your birthday, the license expires automatically, and you cannot legally practice medicine in Arizona.¹

Osteopathic physicians work off January 1 instead of a birthday, but the structure is the same: 30-day grace period before the penalty applies, and four months until automatic expiration. The AZDO may grant an extension to May 1 for physicians who missed CME hours because of disability, military service, absence from the country, or other circumstances beyond their control.²

Renewal Fees

The AMB charges allopathic physicians a $500 biennial renewal fee. Renew more than 30 days after your birthday and the total climbs to $850: the $500 fee plus a $350 late penalty. If your license lapses entirely, the AMB charges a separate $500 reactivation fee on top of whatever the reinstatement process requires.³

The AZDO charges osteopathic physicians $636 for biennial renewal, with a $175 late penalty.⁴ Both boards accept payment electronically through their online portals.

Continuing Medical Education

Both MDs and DOs must complete at least 40 hours of continuing medical education during each two-year renewal cycle.⁵² The specific content requirements are set by each board’s rules. The AMB requires that three of the 40 hours cover opioid prescribing.

You do not submit proof of CME with your renewal application. The AMB randomly audits at least 10 percent of physicians every renewal cycle, and if you are selected you must produce documentation of all 40 hours.⁵ Failing an audit without adequate cause is grounds for probation, suspension, or revocation. Keep your certificates and attendance records for the full cycle even if you are never audited.

What the Renewal Application Asks

The application walks you through a series of disclosure questions covering disciplinary actions, license restrictions, and sanctions imposed by any state licensing board or federal agency. Answer every one. A blank creates a deficiency notice, and if you do not resolve deficiencies within 60 days, your license expires. A “yes” answer requires you to attach an explanation and supporting documents identifying the sanctioning body, the nature of the action, and the underlying charges.¹ Knowingly making a false or misleading statement on a board form is itself unprofessional conduct.

Beyond disclosures, the allopathic renewal checklist typically includes a government-issued photo ID if the board doesn’t already have a legible copy on file, current proof of immigration status, an attestation that you completed a board-prescribed training unit on the Medical Practice Act and board rules,¹ a review of your physician profile on the AMB website with corrections submitted for any inaccuracies, and an attestation that you have a plan for secure storage, transfer, and access of patient records if your practice closes.

DEA Registration and the Prescription Monitoring Program

If you hold an active DEA registration in Arizona, state law requires you to also maintain an active account with the Arizona Controlled Substances Prescription Monitoring Program (CSPMP). No specialty or practice setting is exempt.⁶ If you have more than one DEA number, all of them must be entered in your CSPMP registration. Failure to register can trigger board action.⁷

Federal law, separately, requires a one-time eight-hour training on opioid and substance use disorders for anyone obtaining or renewing a DEA registration. The requirement took effect June 27, 2023, under the Medication Access and Training Expansion (MATE) Act. When you renew your DEA registration, you check a box affirming completion. The hours can be accumulated across multiple courses and can include past training that covered the subject matter. Physicians board-certified in addiction medicine or addiction psychiatry are exempt.

How to Submit Your Renewal

Allopathic physicians renew through the AMB’s online physician portal. The AMB is moving to a new portal system in April 2026, with the licensing system offline from April 3 through April 8. Any application or renewal not paid or completed by April 2 must be restarted in the new portal, so if your birthday falls near that window, allow extra time.⁸ Osteopathic physicians submit through the AZDO’s separate system.

The workflow is: log in, verify your personal and practice information, answer the disclosure questions, confirm your CSPMP registration status if you hold a DEA number, and pay the fee. You can save progress, but the application must be completed and paid before your deadline. A confirmation receipt goes to you once the submission is accepted.

What Happens if You Miss the Deadline

The consequences escalate on a fixed timeline. For allopathic physicians:

Birthday through 30 days after: renew at the standard $500 fee with no penalty.
Day 31 through four months past your birthday: renew for $850 total, which includes the $350 late penalty.
After four months: the license expires automatically, and you cannot legally practice medicine in Arizona.¹

For osteopathic physicians, the same structure runs from the January 1 deadline: 30-day grace period before the $175 penalty applies, and four months to automatic expiration.²

Once a license has expired, you cannot submit a late renewal to bring it back. You have to reapply under the standard application process, with the additional paperwork, fees, and board review that come with it.¹ The board has broad discretion over what it requires, and reapplication can involve demonstrating current competency.

Renewing Through the Interstate Medical Licensure Compact

Arizona participates in the Interstate Medical Licensure Compact (IMLC). If your Arizona license was originally issued through the Compact, you can renew it through the IMLC portal instead of directly with the AMB. This route is only available for licenses issued through the Compact; other Arizona licenses renew through the AMB or AZDO as usual.⁹

The IMLC charges a $25 application fee per renewal in addition to the state renewal fee. You do not need an active Letter of Qualification to renew. Once you submit, the issuing state board verifies your information and determines whether you qualify. You remain responsible for tracking renewal dates and fees for every state license you hold through the Compact.⁹

Practicing on an Expired License

Practicing medicine in Arizona after your license expires violates the Medical Practice Act.¹ The AMB’s primary duty is protecting the public from unlawful and unqualified practitioners, and it can initiate investigations on its own motion.¹⁰ Consequences range from formal reprimand to suspension or revocation depending on the facts.

The downstream effects reach beyond the board itself. An expired or disciplined license can disrupt hospital credentialing, insurance panel participation, and Medicare or Medicaid enrollment. The federal Office of Inspector General can exclude individuals from participating in federal healthcare programs in cases involving fraud or abuse, and board disciplinary actions are reported to national databases accessible to other states.¹¹ An administrative oversight that starts with a missed birthday can become a much larger problem if patient care continues in the meantime.
