In Missouri, no age by itself requires a senior to take a driving test. Turning 70 changes how you renew — in person, every three years, with a vision screening and a road sign recognition test — but it does not put you behind the wheel for an exam. The Department of Revenue can order a full driving test at any age, but only after a credible report that a specific driver may be unsafe.
What Changes When You Turn 70

Online renewal is not an option once you cross that line. Missouri limits remote renewal to drivers between 21 and 49, so anyone 70 or older has to appear at an office every cycle.2Missouri Department of Revenue. Driver Licensing Checklist

The Two Tests You Actually Take at Renewal
Every Missouri renewal, at every age, includes a vision screening. Score 20/40 or better in either eye or both together and you pass with no vision-related restrictions.4Missouri Department of Revenue. 12 CSR 10-24.090 Missouri Driver License or Permit Vision Test Guidelines
If your acuity is below 20/40 but at least 20/160, and your combined peripheral vision is at least 70 degrees, the state can still issue you a license with restrictions. Common ones include corrective lenses, daylight-only driving, a 45 mile-per-hour speed cap, or required outside mirrors. Below 20/160 corrected, or peripheral vision narrower than 70 degrees, the license will not be issued.
Drivers 70 and older take a second test at each renewal: a road sign recognition test. It confirms you still identify standard traffic signs on sight. Neither the vision screening nor the sign test puts you behind the wheel. A routine senior renewal in Missouri does not include a driving exam.
What to Bring to Your Renewal
Bring original documents proving your identity and date of birth, lawful status, Social Security number, and Missouri residency. There is a break for older applicants. If you are 65 or older and previously held a Missouri license, the residency documents are waived at renewal.6Missouri Revisor of Statutes. Once your paperwork clears and you pass the vision and sign tests, the office prints a temporary paper license. The permanent card arrives by mail.
When the DOR Can Order a Full Driving Test
Age is never the trigger. The DOR’s authority to require a road test or medical evaluation runs through Missouri statute 302.291, which lets the director act when there is good cause to believe a driver may be unsafe.7Missouri Revisor of Statutes. Reports usually come from a health care professional, law enforcement, or family. Missouri authorizes physicians, chiropractors, registered nurses, psychologists, social workers, professional counselors, optometrists, physical and occupational therapists, and emergency medical technicians to report. Immediate family members can also report — spouse, parent, child, grandparent, sibling, grandchild, great-grandparent, aunt, uncle, niece, nephew, or great-grandchild. The director can also investigate on information from other sources. A credible letter from a neighbor describing dangerous driving can prompt a re-examination even though the writer is not on the statutory list.
If the director requires a medical exam, the driver gets an initial 30 days to complete it, with extensions available for good cause. When a physician’s report shows a condition making the driver unsafe, the current license is revoked ten days after notice unless the driver produces medical evidence of a change in condition.9Missouri Department of Revenue. The One Age-Based Skills Test: School Bus Endorsement at 75
One narrow exception exists. Drivers 75 or older who hold an “S” endorsement to operate school buses must pass a behind-the-wheel skills exam every two years to keep that endorsement. Letting Your License Lapse More Than 184 Days
Missing your renewal by too much undoes the exemption from road testing. If a Missouri license has been expired more than 184 days, the state treats you like a new applicant: written knowledge test, vision screening, road sign recognition test, and a behind-the-wheel driving exam.10Cornell Law School. 12 CSR 10-24.190 – Driver License Retesting Requirements After a License Expires
This is the most common way older drivers end up taking a road test in Missouri. Someone stops driving during a health episode, the license expires, and by the time they feel ready to drive again the 184-day window has closed. The clock runs from the printed expiration date, not from the last day you actually drove. If you think you might want to drive again within the next year or two, renewing on time keeps you out of a full re-examination.
