Autonomous Practice APRN in Florida: Insurance, Fees, and Renewal

Autonomous practice for an APRN in Florida is available under Florida Statute 464.0123, which lets an experienced Advanced Practice Registered Nurse work without a supervising physician or a written protocol once the Board of Nursing accepts the application. Qualifying comes down to three things: 3,000 supervised clinical hours, specific graduate coursework, and a clean license with the right malpractice coverage in place.

Who Qualifies

The central requirement is 3,000 clinical practice hours completed within the five years before you apply, under the supervision of an M.D. or D.O. Clinical instructional hours count toward that total.1Florida Senate. Florida Code 464.0123 – Autonomous Practice by an Advanced Practice Registered Nurse

You also need graduate-level coursework in two subjects, each completed in the past five years:

  • Three graduate semester hours in differential diagnosis, or 45 practitioner-level continuing education hours.
  • Three graduate semester hours in pharmacology, or 45 practitioner-level CE hours. The pharmacology content must cover the therapeutic uses and biological effects of drugs.

If your MSN or DNP covered these subjects but you graduated more than five years ago, the coursework has to be redone through equivalent classes or the 45-hour CE option in each subject.

Your Florida APRN license must be active, current, and without discipline. Any disciplinary action in Florida or another state within the past five years disqualifies you.2Florida Legislature. Florida Statutes 456.072 – Grounds for Discipline, Penalties, Enforcement You also need active national certification in your specialty role, which is already a prerequisite for the underlying APRN license.1Florida Senate. What Autonomous Registration Lets You Do

Registration removes the supervising-physician requirement and the written protocol. You gain legal authority to diagnose independently, order and interpret diagnostic tests, and manage treatment plans within your specialty.1Florida Senate. Florida Code 464.0123 – Autonomous Practice by an Advanced Practice Registered Nurse

One important boundary: autonomous practice in Florida is limited to primary care, defined by statute as family medicine, general pediatrics, and general internal medicine.3The Florida Statutes. Florida Statutes 464.0123 – Autonomous Practice by an Advanced Practice Registered Nurse If you practice acute care, dermatology, or another non-primary-care specialty, autonomous registration does not extend to that work.

Prescribing Under Autonomous Status

Autonomous APRNs keep the prescribing authority available to any APRN with a master’s or doctoral degree and specialized practitioner training, which includes controlled substances in Schedules II through V. Schedule II prescriptions are capped at a 7-day supply. Psychiatric nurses prescribing psychiatric medications for mental health disorders are not subject to that 7-day limit.4The Florida Statutes. Florida Statutes 464.012 These prescribing rules apply to all APRNs with prescriptive authority, not only those registered as autonomous.

Malpractice Insurance You Have to Carry

Florida requires APRNs to maintain professional liability coverage as a condition of licensure and renewal. The minimum is $100,000 per claim with a $300,000 annual aggregate.5Florida Board of Nursing. Board of Nursing Financial Responsibility You can meet it through a commercial policy from an authorized insurer, a surplus lines insurer, a risk retention group, the Joint Underwriting Association, or a qualifying self-insurance plan.

The practical shift matters. When you work under supervision, your employer’s policy typically covers you. Autonomous, you carry your own. You must submit the Board’s financial responsibility form at initial licensure and at every renewal.5Florida Board of Nursing. Board of Nursing Financial Responsibility

  • Proof of current national certification in your specialty role.
  • The financial responsibility form confirming malpractice coverage meets the $100,000/$300,000 minimum.

Fingerprinting

Following the 2024 legislative session (HB 975), electronic fingerprinting is mandatory for autonomous APRN registration, and the application cannot be approved until the background screening clears.6Florida Board of Nursing. Fees

The initial APRN application and licensure fee is $110.9Florida Board of Nursing. Advanced Practice Registered Nurse The Board does not list a separate fee for the autonomous registration itself, so your main costs are the APRN license fee (if you’re not already licensed), fingerprinting, and malpractice insurance. If your APRN license is already active, you’re adding autonomous registration to it rather than paying licensure again.

What the State Application Doesn’t Handle

Board approval covers the state clinical authority. Independent prescribing and billing require separate federal steps.

Any practitioner prescribing controlled substances needs an individual DEA registration. If your prescribing previously ran through a practice’s DEA number, you’ll need your own, tied to each address where you handle controlled substances. The DEA classifies nurse practitioners as mid-level practitioners authorized to prescribe controlled substances in states that permit it.10DEA Diversion Control Division. Mid-Level Practitioners Authorization by State

On the billing side, an individual NPI (Type 1) may need updated taxonomy codes and a new practice location through NPPES; if you’re forming a business entity, that entity also needs an organizational NPI (Type 2).11NPPES. Apply for an NPI Direct Medicare billing goes through PECOS or Form CMS-855I, and your NPI must be in place first.12CMS. Medicare Enrollment for Physicians, Non-Physician Practitioners and Other Health Care Suppliers Form CMS-855R handles reassignment if another entity bills on your behalf. Florida Medicaid runs its own provider agreement and screening.13Medicaid.gov. Medicaid Provider Enrollment Compendium Credentialing with private insurers is separate again, and 90 to 120 days is a common lead time. Start these enrollments well before you see your first independent patient.

If you’re setting up your own practice, that means an EIN from the IRS, a Florida business license, and a business entity structure (Florida APRNs commonly organize as an LLC or PLLC). If you’ll perform even waived in-office tests like a rapid strep or urinalysis dipstick, the practice location needs a CLIA certificate of waiver from CMS.

Keeping the Registration Active

Autonomous registration renews on the same biennial cycle as the standard APRN license, and the Board currently staggers expiration dates across three groups.14Florida Board of Nursing. Advanced Practice Registered Nurse Renewal The license must stay active and discipline-free through the cycle.

Autonomous APRNs owe 10 additional hours of graduate-level continuing education each renewal cycle, on top of standard APRN CE. These can be nurse practitioner-level courses from a professional or national nursing specialty board, or continuing medical education credits.14Florida Board of Nursing. Advanced Practice Registered Nurse Renewal All CE, including the additional 10, is tracked through CEBroker.

At renewal, you also update your practitioner profile through MQA Online Services, confirm your primary practice address, and resubmit the financial responsibility form showing malpractice coverage still meets the minimums.14Florida Board of Nursing. Advanced Practice Registered Nurse Renewal A lapse in any of these puts autonomous status at risk.
