California CDPH license renewal happens through the branch that issued your credential, on a schedule that depends on what you hold: clinical laboratory personnel move to a one-year cycle starting January 1, 2026, Certified Nurse Assistants stay on two years, and facility licenses renew every 12 months with a hard 30-day pre-expiration filing deadline. Miss the deadline and you cannot legally practice or operate until the credential is restored, and if you bill federal programs the fallout can extend well past the state fee.
Which Credential You Hold Determines Everything
The California Department of Public Health issues two broad categories of credentials, and each follows its own rules. On the personnel side, CDPH directly certifies or licenses nurse assistants, home health aides, hemodialysis technicians, and nursing home administrators, and licenses phlebotomy technicians and clinical laboratory scientists through its Laboratory Field Services (LFS) branch.1CDPH. The 2026 Change for Lab Personnel
If you hold any clinical laboratory personnel license or certificate, this is the change to plan around. Beginning January 1, 2026, all lab licenses and certificates shift to a one-year renewal period. Your printed license may still show a two-year expiration date because of ongoing IT system upgrades, but CDPH requires you to renew after one year regardless of what the document says.2CDPH. CNA certificates and facility licenses are not affected. CNAs still follow a two-year cycle, and facility licenses continue to renew every 12 months.
When to File
Facility timing is set by statute and unforgiving. Each facility license expires 12 months from its issuance date, and CDPH’s License Renewal and Certification Branch sends a renewal application with an invoice 120 days before that date.3CDPH. For lab personnel, your continuing education must be completed within the 12 months before you submit the renewal application, so plan CE around your expiration date rather than the calendar year.
Continuing Education Required Before You Renew
Nearly every CDPH credential requires CE or in-service training completed before renewal, and the hours vary sharply by license.
Certified Nurse Assistants must complete 48 hours of in-service training or CE units per two-year certification period, with at least 12 hours completed in each year of the cycle. Certified Hemodialysis Technicians need 30 hours of in-service training or CE units in dialysis care or general patient care during the certification period.6CDPH. Certified Hemodialysis Technician Renewal Application
Registered Environmental Health Specialists need 24 contact hours every two years through agencies approved by the CDPH REHS Program, and excess hours do not carry over. All CE must come from CDPH-approved providers. Keep certificates showing the provider name, course title, and credit hours. CDPH recommends retaining documentation for at least four years in case of an audit.8CDPH. Background Checks and Address Updates
Some credentials require current criminal record clearance at renewal. CNAs must maintain that clearance, and CDPH provides a sample Live Scan form specifically for nurse assistant renewals.9CDPH. Before submitting any renewal, confirm your mailing address and contact information are current. Facility renewals must reflect any changes in ownership or affiliation before filing. Facility Fees
General acute care hospitals pay a per-bed statewide license fee of $796 for fiscal year 2025–26. Los Angeles County facilities pay an additional $180 per bed on top of that.13CDPH. How to Submit Online
CDPH no longer accepts paper applications for lab personnel, and the correct portal depends on your branch.
Clinical laboratory personnel renew through the Laboratory Field Services online system. Log in (or create an account), open your business dashboard, find the State ID for the license you’re renewing, and start the renewal application. Facility licenses use the CDPH Portal, which currently handles General Acute Care Hospital and Acute Psychiatric Hospital applications online.15CDPH. CDPH Portal Home Select the renewal option for your existing license rather than starting a new application, because a new application can generate a new State ID and create a gap in your licensure history.
Each portal will ask you to verify pre-populated information, attest to completed CE, upload supporting documents, and pay electronically. Save the confirmation number and receipt.
If You Miss the Deadline
California law prohibits practicing under an expired credential. What it takes to get back in good standing depends on the license and how late you are.
Lab Personnel
Renew after your expiration date and you pay a delinquency fee on top of the standard renewal. Under the 2026 schedule, the delinquency fee for most license types equals the full annual renewal fee. CNAs
A CNA whose certificate has been lapsed for less than two years can reactivate by passing the Competency Evaluation. Submit renewal application CDPH 283C, check the “yes” box in the Reactivation section, and CDPH sends an approval letter with scheduling instructions. The evaluation must be completed within two years of the expiration date.16CDPH. REHS Professionals

Facilities
A facility that fails to file at least 30 days before expiration faces license expiration under Health and Safety Code section 1267. An expired facility license means the facility cannot legally operate or bill for services. Reinstatement typically requires a new application rather than a late filing, which can involve additional inspections and significant delays.
Federal Consequences of Practicing on an Expired License
State-level penalties are only part of the picture if you bill Medicare, Medicaid, or other federal programs.

Medicare enrollment carries a separate risk. A provider’s Medicare enrollment can be revoked if the state license is suspended, revoked, or surrendered. Facilities that employ someone with an expired credential face civil monetary penalties if that person provides services payable by a federal program. The OIG can impose penalties of up to $10,000 per item or service claimed, plus an assessment of up to three times the amount claimed.22CMS.
