California Paid Family Leave Form (DE 2501F): Filing and Denials

To file for California Paid Family Leave, you complete the claim form DE 2501F either online through the EDD’s SDI Online portal or on paper mailed to the EDD. Online filing is faster, the form is split into parts filled out by different people depending on your reason for leave, and you have 41 days from the first day of your leave to submit it.

How to Get the DE 2501F

The quickest route is to skip the paper form and file through SDI Online at edd.ca.gov. The system walks you through each section and lets you upload supporting documents directly.

If you want or need paper, you have a few ways to get one. You can order a DE 2501F through the EDD website and expect about ten days for delivery, ask your doctor or employer for a copy, visit a local SDI office, or call 1-877-238-4373.1Employment Development Department. If you’re a new mother transitioning from a pregnancy-related disability claim, you don’t need to request anything. The EDD automatically sends you a specialized form, the DE 2501FP, when your disability payments end. What to Have Ready Before You Start

Gather everything before you sit down with the form. Chasing documents mid-application is the easiest way to introduce errors or miss a deadline. You’ll need your Social Security number, your most recent employer’s name and address, and the last date you worked or plan to work before your leave begins.

  • Bonding claims need a proof-of-relationship document: a birth certificate (hospital or official), a Declaration of Paternity, an Adoptive Placement Agreement, a foster care agency letter, or similar evidence showing your relationship to the child.
  • Care claims require the care recipient’s physician or practitioner to certify the serious health condition and the need for care. The care recipient also has to sign a section authorizing the claim. Coordinate this in advance so you’re not chasing signatures after your own section is done.
  • Military assist claims need both military documentation (active duty orders or a letter of impending call to duty, for example) and documentation of the qualifying event itself, such as a meeting announcement for a military briefing or a bill for related legal or financial services.

Who Can File

PFL is funded through State Disability Insurance payroll deductions that show up as “CASDI” on your pay stub, and if you’re a W-2 employee in California with those deductions, you’re already paying in.4Employment Development Department. Paid Family Leave To qualify for benefits you need at least $300 in SDI-taxable wages during your base period, and you must be losing wages because you’re taking time off for one of three reasons: bonding with a new child within 12 months of birth, adoption, or foster placement; caring for a seriously ill family member (child, parent, spouse, registered domestic partner, grandparent, grandchild, sibling, or parent-in-law); or participating in a qualifying event related to a family member’s military deployment to a foreign country.5Employment Development Department. Self-employed workers and independent contractors are not automatically covered, but can opt in through the EDD’s Disability Insurance Elective Coverage program.6Employment Development Department. Disability Elective Coverage Benefits and Premium Amounts

Filling Out the Form Part by Part

The DE 2501F is divided into sections completed by different people. Which sections apply depends on your reason for leave.

Part A: Statement of Claimant

Every claimant fills out Part A. You give your personal details, employment information, and the specifics of your leave, including why you need it and when it starts. Answer every question carefully and double-check dates. Errors in Part A are the most common cause of processing delays.

Part B: Bonding Certification

If you’re filing a bonding claim, you complete Part B and attach your proof-of-relationship document.7Employment Development Department. Guide for Completing a Claim Form for Paid Family Leave Benefits Birth verification must clearly show the child’s name, date of birth, and the name of the parent claiming benefits. For foster care or adoption, the document has to include the date the child was placed in your custody.

Parts C and D: Care Recipient and Physician

For care claims, the care recipient completes and signs Part C, authorizing the claim. Their physician or practitioner completes and signs Part D, certifying the medical need. You cannot fill out these sections yourself. They require the care recipient’s and doctor’s own signatures, so give them time.

Part E: Military Assist

For a military assist claim, you complete Part E with information about the family member’s deployment and the qualifying event, and attach the supporting military and event documentation.

When to Submit and How Long It Takes

Your filing window opens on the first day your leave begins and closes 41 days later. Filing before your leave starts will cause problems, and filing after the 41-day window risks losing benefits.8Employment Development Department. Paid Family Leave – Step 2 Apply

If you file online, you can upload all supporting documents digitally and get faster processing. If you’re mailing paper, send the DE 2501F with all completed sections and attachments together. Don’t send Part A now and Part D later; incomplete packages sit in limbo until everything arrives.

PFL benefits can be taken intermittently rather than all at once. You could use three weeks now and five weeks later, as long as you continue to meet eligibility rules and stay within the allowed timeframe for your claim type.9Employment Development Department. The EDD typically processes a completed claim and issues your first payment within about two weeks.10Employment Development Department. Step 5: Receive Your First Payment Unlike state disability insurance, PFL has no waiting period; benefits start from the first day of your qualifying leave.

You choose how to receive payments when you file. Options are direct deposit (online filers only), a Money Network prepaid debit card, or a mailed check. If the EDD needs more information, they’ll send a request. Respond quickly. Delays in getting back to the EDD can stall or kill your claim.

PFL Does Not Protect Your Job

One boundary matters enough to state directly before you file: PFL is a wage replacement program, nothing more. It sends you partial pay while you’re off work. It does not guarantee your employer will hold your position or refrain from replacing you.

Job protection comes from separate laws. The California Family Rights Act (CFRA) gives eligible employees up to 12 weeks of job-protected leave per year for bonding with a new child or caring for a seriously ill family member, and requires the employer to maintain health benefits during that time. The federal Family and Medical Leave Act (FMLA) provides similar protections. Not everyone qualifies for CFRA or FMLA; both have minimum requirements tied to how long you’ve worked for your employer and how many employees the company has. Many workers take PFL and CFRA or FMLA leave at the same time: one provides the paycheck, the other protects the job. Filing the DE 2501F on its own does not activate any job protection, so confirm your CFRA or FMLA eligibility with your employer’s HR department before your leave starts.

If Your Claim Is Denied

If the EDD denies your claim, you have 30 days from the mailing date on the denial notice to appeal.12Employment Development Department. Appeal Form (DE 1000M) The appeal goes on Form DE 1000M, completed in black ink and mailed to the EDD office listed on your denial notice.

On the form, explain why you disagree with the decision. Be specific. “I disagree” isn’t enough. Describe what the EDD got wrong and what evidence supports your position. If you need more space, attach extra pages with your name and Social Security number on each. An administrative law judge will schedule a hearing to review the case.

If you miss the 30-day deadline, you can still file, but you must explain why you were late. The judge decides whether your reason counts as good cause; if it doesn’t, the appeal is dismissed. While your appeal is pending, keep certifying for benefits if you’re receiving any. Stopping certification can create additional problems.
