You cannot submit a DOT physical online in Florida, because the exam itself must be done in person by a certified medical examiner. What has changed is what happens after the exam: as of June 23, 2025, your examiner sends the results electronically to FMCSA, which passes them to the Florida Department of Highway Safety and Motor Vehicles (FLHSMV) automatically. The only piece of the process you handle online is self-certifying your type of commercial driving operation.
Why the Exam Has to Be In Person
A DOT physical is hands-on. The medical examiner checks your vision, hearing, blood pressure, heart and lung function, reflexes, range of motion, and other systems, then records the findings on the Medical Examination Report Form (MCSA-5875). How Your Results Now Reach Florida
Before June 2025, Florida CDL holders had to submit a copy of the paper certificate themselves, either online through the FLHSMV or in person at a driver license office. That step is gone. Since June 23, 2025, the medical examiner electronically transmits your exam results to FMCSA’s National Registry, and FMCSA forwards them to the FLHSMV.4Florida Department of Highway Safety and Motor Vehicles. Because the transfer is automated, the details on your Medical Examination Report Form have to match your Florida driver’s license exactly. Your name, date of birth, licensing state, and license number all need to line up. If any of those fields is off, the transmission can fail and your certification will not reach the FLHSMV.5Federal Motor Carrier Safety Administration. The FLHSMV suggests carrying your paper Medical Examiner’s Certificate for at least 15 days after the examiner submits results electronically, in case the update has not posted yet to your driving record.4Florida Department of Highway Safety and Motor Vehicles. CDL Medical Information After that, you are no longer required to carry one.
The Online Step: Self-Certification
Every Florida CDL holder still has to self-certify their type of commercial driving operation. This is separate from the medical exam, and it is the piece you can complete online through the FLHSMV. - Non-excepted interstate. You drive or expect to drive across state lines and must keep a current federal medical certificate. Most long-haul drivers fall here.
- Excepted interstate. You drive across state lines but only in activities that are exempt from the federal medical certificate requirement, such as transporting school children or operating certain government vehicles.
- Non-excepted intrastate. You drive only within Florida and must meet the state’s medical certification requirements.
- Excepted intrastate. You drive only within Florida in activities that exempt you from the state’s medical certification requirements.
If you drive in both excepted and non-excepted operations, you have to certify in the non-excepted category.6Federal Motor Carrier Safety Administration. Drivers in the excepted categories do not need a medical certificate but still have to self-certify.
Confirming the FLHSMV Received Your Results
After the exam, check that your medical certification actually posted to your Florida driving record. You can look through the FLHSMV’s online portal or call the CDL Helpdesk at 850-617-2606. If nothing appears after that window, the problem is usually a data mismatch on the Medical Examination Report Form. Compare the name, date of birth, and license number on the form to your Florida license. If you spot a discrepancy, contact your medical examiner so they can correct the record and retransmit it.
What Happens If You Let It Lapse
Letting the medical certificate expire in Florida is not just paperwork. The FLHSMV will disqualify you from operating a commercial vehicle, and your CDL must be surrendered for a non-commercial Class E license until you update your medical certification or switch your self-certification to an excepted category.7FMCSA. Getting your CDL privileges back means obtaining a new medical certificate and having it on file with the FLHSMV. Florida may also require additional fees or retesting before reinstating your CDL.8Federal Motor Carrier Safety Administration. Why a Paper Copy Still Matters
Even though the state filing is now electronic, keep the paper certificate or a PDF after each exam. You may need it during the 15-day window before the update posts. If you drive for a motor carrier, your employer also has to keep a copy in your driver qualification file under federal rules, and the electronic transmission to Florida does not satisfy that separate obligation. Send your employer a current copy after every exam.
