Check every field before it goes in the envelope. A wrong VIN digit, a misspelled name, or a missing lien release will bounce the application back, and each round trip adds weeks.
In Person
Bring the completed form, your documents, and payment to any Driver Services facility. Staff can review the paperwork on the spot and flag problems before it enters the processing queue. You still will not walk out with the title itself.

Who Can Apply
Only the registered owner can apply for a duplicate. If the vehicle has more than one owner, every listed owner must sign. To let someone else handle it, use the Motor Vehicle Power of Attorney (Form RT-5) to authorize a representative to sign title documents for you.
An active lien changes things. You need a release letter from your lender before a clean duplicate title can be issued in your name. If you apply while the lien is still on record, the title is mailed to the lienholder rather than to you.7Official Website of the Illinois Secretary of State. Mistakes That Get Applications Rejected
A few errors account for most kickbacks:
- A single wrong digit in the 17-character VIN will trigger a rejection. Copy it directly from your registration card or the dashboard plate.
- The name on your application must match the state’s record exactly. If your name has changed, include legal documentation of the change.
- Submitting without a lien release when the state’s records still show an active loan is one of the fastest ways to get your paperwork returned.
- Blank fields slow things down. Fill in every section that applies, including the odometer reading, which people often skip.
If You Never Received a Title From the Seller
A duplicate title only works if you are already the registered owner. If you bought a vehicle without a title and cannot reach the seller, Illinois handles that situation through bonded titles under 625 ILCS 5/3-109. A bonded title is backed by a surety bond covering one and a half times the vehicle’s appraised value, and the bond stays active for three years.
You have to document how you got the vehicle with a bill of sale, receipt, canceled check, or notarized statement, and you need an independent appraisal from a licensed dealer, rebuilder, or other qualified professional. Self-appraisals are not accepted. The application still uses Form VSD 190, but the fee is $95 rather than $50, and you submit it at a Secretary of State facility.
