If you are upgrading to a REAL ID, or your name or address has changed, you need supporting documents: proof of identity (a certified U.S. birth certificate, valid U.S. passport, or citizenship/naturalization certificate), proof of Social Security number, two proofs of Kentucky residency dated within the last year, and, if your name has changed, a certified marriage certificate, divorce decree granting the change, or legal name change order. To use it, all of the following must be true: your license is not expired by more than one year, you are a U.S. citizen, you are at least 21 (or have turned 21 since your last license was issued), and you have completed the vision screening with either a one-time passcode from a regional office visit or a signed certification form from a vision specialist.9Kentucky Drive.
