Yes, AHCCCS does cover sports physicals, but only for members under 21 and only when the exam is done as part of an EPSDT well-child visit. For adults 21 and older, sports physicals are explicitly excluded. There is no standalone sports physical benefit at any age.
Getting a Sports Physical Covered for a Child Under 21
Schedule a well-child visit with your child’s primary care provider and tell the office you also need a sports clearance form filled out. For Arizona high school athletes, the AIA requires four: a parent/student evaluation form, a medical examination form, an acknowledgement form, and a consent-to-treat form.2AIA Online. Why It Has To Be a Well-Child Visit
AHCCCS doesn’t reimburse a sports physical as a service on its own. The exam is folded into the annual well-child visit under the Early and Periodic Screening, Diagnostic, and Treatment program (EPSDT), which federal law requires every state Medicaid program to provide to children and young adults under 21.5Medicaid.gov. The two visits overlap almost completely. A well-child exam already checks heart health, blood pressure, vision, hearing, flexibility, bone and muscle health, height, and weight, plus immunizations, developmental screening, behavioral health, and any needed labs.7Arizona Complete Health. Free Community Sports Physical Events
Some Arizona community health centers hold seasonal sports physical clinics that accept AHCCCS. Adults 21 and Older Are Not Covered
For AHCCCS members aged 21 and up, sports and exercise physicals are listed as non-covered services. The AHCCCS Medical Policy Manual excludes “qualifications for sports or physical exercise activities” from adult coverage, along with pre-employment physicals, pilot exams, and disability certification exams.12AHCCCS.
