Arizona Complete Health does cover vision care, but what a member actually gets depends on which plan they hold. The insurer runs three separate lines in Arizona — Medicaid (AHCCCS Complete Care), Medicare Advantage (Wellcare), and ACA Marketplace (Ambetter) — and the gap between them is wide enough that one member may have full eyewear benefits while another has almost none for routine adult vision.
Coverage by Plan Type
- Medicaid (AHCCCS), under 21: full coverage for eye exams, vision screenings, prescription glasses, and replacements, with no referral needed.
- Medicaid (AHCCCS), 21 and older: no routine vision coverage; prescriptive lenses are covered only as a prosthetic device after cataract surgery, with prior authorization.
- Medicare Advantage (Wellcare): routine annual eye exams, glaucoma and diabetic retinal exams, and eyewear allowances, with specifics that vary by plan.
- Marketplace (Ambetter): pediatric vision included in every plan at no charge; adult vision only on plans purchased with the vision add-on.
Medicaid (AHCCCS) Vision Benefits
For AHCCCS Complete Care members under 21, vision is a full benefit under the federal Early and Periodic Screening, Diagnostic, and Treatment program. That includes eye exams, vision screenings, prescription eyeglasses, repairs, replacements for lost or broken glasses, and new lenses when a prescription changes. Adults 21 and older are treated very differently. Routine eye exams and eyeglasses are not on the AHCCCS covered services list for adults.3AHCCCS. Medicare Advantage (Wellcare) Vision Benefits
Arizona Complete Health’s Medicare Advantage plans, sold under the Wellcare brand, generally carry more generous vision benefits. Marketplace (Ambetter) Vision Benefits
On the ACA Marketplace side, vision works differently again. Pediatric vision is a federally required essential health benefit, so every Ambetter plan covers children’s eye exams and glasses at no charge, limited to one visit and one pair of glasses per year.7Ambetter Health. Adult vision is not automatic. Using the Vision Benefit
The vision network for Arizona Complete Health’s Medicaid and Marketplace lines is administered by Centene Vision Services, formerly Envolve Vision.11Centene Vision. To find an in-network eye doctor, use the online provider search for the specific plan or call the vision vendor number on the back of the ID card. Staying in network matters, because out-of-network vision services may not be covered at all, leaving the member on the hook for the full bill. For the dollar amounts tied to any specific plan — the eyewear allowance, copays, or the Spendables balance — the Summary of Benefits or Evidence of Coverage for that plan is the document that governs, available on the plan website or through member services.
