Arkansas Blue Cross and Blue Shield does not cover Zepbound when it is prescribed for weight loss. The insurer treats weight-loss medications as a non-covered benefit across its commercial plans and through its Health Advantage HMO affiliate, and a December 2023 provider bulletin named Zepbound specifically among the drugs excluded from the formulary.1Arkansas Blue Cross and Blue Shield. Why the Exclusion Exists
Zepbound and Mounjaro share an active ingredient, tirzepatide, but their FDA approvals differ. Mounjaro is approved for type 2 diabetes. Zepbound is approved for chronic weight management in adults with obesity and, since December 2024, for moderate-to-severe obstructive sleep apnea in adults with obesity.2FDA. The December 2023 bulletin stated that “weight loss drugs are not a covered benefit for Arkansas Blue Cross and Blue Shield or Health Advantage members,” listing Saxenda, Wegovy, and Zepbound as drugs kept off the formulary because their FDA-approved indications are for weight loss. Mounjaro and Ozempic remain covered when prescribed for type 2 diabetes, but every GLP-1 prescription now requires confirmation of a diabetes diagnosis, either through a diagnosis code on the prescription or through prior authorization with medical records.1Arkansas Blue Cross and Blue Shield. The exclusion is legal because the Affordable Care Act does not require insurers to cover drugs for “anorexia, weight loss, or weight gain” as part of Essential Health Benefits.3HealthInsurance.org. How the Exclusion Applies to Your Plan
The answer is the same across most Arkansas Blue Cross products, but the mechanics differ slightly.
On commercial plans (PPO, HMO, POS, employer-sponsored), Zepbound carries a prior authorization requirement, but the underlying benefit exclusion for weight-loss drugs means prior authorization does not translate into approval for weight-loss use.5PrescriberPoint. ARBenefits, the plan for Arkansas state and public school employees administered by Health Advantage, has historically not covered weight-loss medications.6GW School of Public Health. Arkansas Medicaid restricts GLP-1 coverage to type 2 diabetes and does not cover the drugs for obesity.8KFF. The Sleep Apnea Route
The 2024 FDA approval of Zepbound for moderate-to-severe obstructive sleep apnea in adults with obesity opened a separate clinical indication that some national insurers now treat apart from weight loss. UnitedHealthcare, for example, covers Zepbound for OSA under its clinical pharmacy program even where weight-loss use is excluded, subject to sleep study findings, BMI, and CPAP-related criteria.10UnitedHealthcare. Arkansas Blue Cross has not published a standalone OSA policy for Zepbound. If you carry a qualifying sleep apnea diagnosis, your prescriber can submit a prior authorization or formulary exception request framed around OSA rather than weight management. That is a possibility to test with your plan, not a guaranteed pathway.
Requesting an Exception and Appealing a Denial
The exception process starts with your prescriber. A Letter of Medical Necessity on the provider’s letterhead should include your name and member ID, the diagnosis, therapies you have tried and failed, and supporting records. If the exception is denied, you have 180 days from the denial notice to file a written internal appeal. If the internal appeal fails, you can request an external review through the Arkansas Insurance Department within four months of the denial. The independent reviewer’s decision binds both sides. Standard reviews take up to 45 days; expedited reviews, used when delay could jeopardize your life or health, are decided within 72 hours.11Arkansas Blue Cross and Blue Shield. Because the exclusion is a benefit design decision rather than a medical necessity call, appeals for weight-loss use face long odds. Members with complex clinical pictures beyond weight management, and those with a documented OSA indication, have the strongest arguments.
What Zepbound Costs Without Coverage
Manufacturer list prices run from $499 to about $1,086 for a 28-day supply depending on dose, and average retail pricing for the single-dose pen reaches roughly $1,291 per month.13Eli Lilly and Company. Zepbound Pricing Information Eli Lilly runs several programs that lower those numbers:
- The Zepbound Savings Card lets patients with commercial insurance that excludes Zepbound pay as little as $499 per month for the single-dose pen. This is the path most Arkansas Blue Cross members will use.14Eli Lilly and Company. Zepbound Savings

Patients enrolled in Medicare, Medicaid, TRICARE, VA, or other government insurance are excluded from all Lilly savings programs. The current programs expire December 31, 2026.14Eli Lilly and Company. Zepbound Savings
Where Arkansas Law Stands

Arkansas Blue Cross has said it is exploring options for employer groups that want to add weight-loss drug coverage, including group riders that would require prior authorization or step therapy, while noting any addition would raise premiums for the entire group.4Health Benefit Insight.
