Asking for a Formulary Exception

There are two main submission paths. One important limit: plans that categorically exclude weight-loss drugs generally do not offer appeal rights for a weight-loss prescription. Prior Authorization and Step Therapy

Even on BCBS plans that do cover Wegovy, prior authorization is almost always required. The prescriber has to send documentation showing the patient meets specific clinical criteria before the plan will pay. Across BCBS affiliates, those criteria track the FDA-approved indications:

  • Adults with a BMI of 30 or higher, or a BMI of 27 or higher plus at least one weight-related condition such as high blood pressure, type 2 diabetes, or high cholesterol.
  • Adolescents age 12 and older with a BMI at or above the 95th percentile for age and sex.

Plans commonly ask for documentation that the patient is following a reduced-calorie diet and exercise plan. What Wegovy Costs If Your Plan Won’t Pay

If You’re on Medicaid or Medicare Instead

Two boundaries are worth flagging, because Minnesotans sometimes hold BCBS MN in one context and public coverage in another.

Minnesota Medical Assistance (the state’s Medicaid program) does cover Wegovy under Minnesota Health Care Programs, subject to prior authorization. Approval requires documentation of a reduced-calorie diet and increased physical activity, plus BMI thresholds matching the FDA label: 30 or higher for adults, or 27 or higher with a weight-related condition. Medicare Part D does not cover Wegovy for weight loss alone. It does cover it for a narrower use: reducing the risk of major cardiovascular events in adults with established heart disease who also have obesity or are overweight. Starting July 1, 2026, a CMS demonstration called the Medicare GLP-1 Bridge will offer Wegovy for weight management to qualifying beneficiaries through December 31, 2027, at a $50 copay, with prior authorization tied to BMI and comorbidities such as heart failure, hypertension, chronic kidney disease, or pre-diabetes. A Pending Minnesota Coverage Bill

Minnesota currently has no law requiring private insurers to cover anti-obesity medications.14Axios.
