Blue Cross Blue Shield of Mississippi does not cover Zepbound under its standard plans. The insurer’s GLP-1 agonist medical policy limits medically necessary coverage to four type 2 diabetes drugs, and Zepbound is not among them.1BCBS Mississippi. For everyone else, manufacturer programs from Eli Lilly are the most reliable way to bring the cost down.
Why the Standard Policy Excludes Zepbound
BCBS Mississippi’s GLP-1 agonist policy (L.5.01.428) names four medications as potentially medically necessary: Mounjaro, Ozempic, Rybelsus, and Trulicity. All four are approved for type 2 diabetes. The policy’s medical necessity criteria require a confirmed type 2 diabetes diagnosis, typically an A1C of 6.5% or higher, and the code reference section contains no diagnosis codes for obesity or chronic weight management.1BCBS Mississippi. There is no line in the policy that says “Zepbound is excluded” in those words. The exclusion works structurally: by tying medical necessity entirely to type 2 diabetes, the policy has no pathway to approve a drug whose only approved uses are weight management and obstructive sleep apnea. That result holds regardless of the indication a doctor writes on the prescription.
The policy itself flags one caveat. Medical policy guidelines do not override a member’s specific benefit plan, and coverage can differ plan by plan. Employer Groups That Have Added Zepbound
Self-insured employers pay their own claims and can customize their formularies beyond the standard policy. Two Mississippi employer groups administered through BCBS Mississippi have done exactly that:

If you get insurance through an employer, it is worth calling your benefits administrator or logging into the BCBS Mississippi formulary tool with your member ID before assuming Zepbound is out. Employer coverage decisions of this kind are not always well-publicized to members.
State and School Employees
Mississippi state and school employees are on a separate plan administered in part by CVS Caremark as the pharmacy benefit manager, and the 2026 plan document lists Zepbound in its Prescription Drug Program.2Mississippi Department of Finance and Administration. 2026 Plan Document This is a change from the plan’s older stance. The prior authorization bar is high. Under CVS Caremark’s criteria for the state plan, a patient must have a BMI of 30 or higher, or 27 or higher with a weight-related condition such as hypertension or type 2 diabetes. Before drug therapy begins, the patient must have participated in a comprehensive weight management program for at least six months, and must use the medication alongside a reduced-calorie diet and increased physical activity. To continue therapy, the patient has to show at least a 5% loss of baseline body weight, or maintenance of the initial loss, after at least three months on a stable dose.4CVS Caremark. Mississippi Medicaid Does Not Cover Zepbound Specifically
Mississippi Medicaid started covering GLP-1s for obesity in 2023, one of only 13 state programs doing so as of January 2026.6KFF. Zepbound is not listed among covered anti-obesity medications in the current prior authorization criteria.7Mississippi Division of Medicaid. Cost Options When Your Plan Will Not Pay
Eli Lilly runs several programs that can reduce the out-of-pocket price of Zepbound. Which one applies depends on your insurance situation.
- If you have commercial insurance that covers Zepbound, the manufacturer savings card can bring the cost to as little as $25 for up to a three-month supply, subject to an annual savings limit of $1,300.8Eli Lilly – Zepbound. Zepbound Savings

- Paying cash through LillyDirect, the KwikPen starts at $299 per month for 2.5 mg, $399 for 5 mg, and $449 for 7.5 mg through 15 mg, provided you refill within 45 days of the previous purchase. Miss that window and the price can climb to $699 at higher doses.9Eli Lilly. LillyDirect – Zepbound
- The Lilly Cares Foundation offers certain Eli Lilly medications free for up to 12 months to qualifying patients with financial need. Check the current list of covered medications to confirm Zepbound is included.10Eli Lilly. Lilly Cares Foundation Patient Assistance Program
Two boundaries on these programs matter. People with government insurance, including Medicare, Medicaid, TRICARE, and VA plans, are not eligible for the savings card.8Eli Lilly – Zepbound. Zepbound Savings And some BCBS Mississippi plans route specialty drugs through an Alternate Funding Program or a specialty vendor, which can disqualify members from using manufacturer coupons. Check your plan documents before assuming a coupon will apply.
Appealing a Denial
Providers can submit a prescription drug prior authorization appeal through the BCBS Mississippi online portal.11BCBS Mississippi. An appeal based on additional clinical information has a chance if the medication would otherwise meet the policy’s medical necessity criteria. An appeal against a formulary exclusion is harder, because the policy explicitly states that a prescription alone does not establish medical necessity and that medical policy does not override plan language.1BCBS Mississippi. The Broader Insurer Retreat
BCBS Mississippi’s approach reflects a wider pullback across Blue Cross plans. Blue Cross Blue Shield of Massachusetts ended coverage of weight-loss GLP-1s, including Zepbound, Wegovy, and Saxenda, upon plan renewal starting January 1, 2026, describing them as “far from being cost effective” at current prices.12BCBS Massachusetts. The financial pressure driving these decisions is documented. An Employee Benefit Research Institute study, funded in part by the BCBS Association, estimated that broad GLP-1 coverage at real-world adherence rates could raise employer premiums by roughly 10%, and by nearly 14% at full uptake.15EBRI. For Mississippi members, the practical takeaway is straightforward. Check whether your specific employer group has added Zepbound.
