Blue Cross Blue Shield of Michigan does cover GLP-1 medications when they are prescribed to treat Type 2 diabetes, but it has eliminated coverage of GLP-1 drugs prescribed for weight loss on most of its commercial plans. Whether your prescription is paid for comes down to two things: the diagnosis on the prescription and the type of plan you have.
GLP-1s Prescribed for Type 2 Diabetes
BCBSM and its HMO affiliate, Blue Care Network, continue to cover Ozempic (semaglutide), Mounjaro (tirzepatide), Rybelsus (oral semaglutide), Trulicity (dulaglutide), and liraglutide when they are prescribed for Type 2 diabetes.1BCBSM MiBluedaily. Two older diabetes GLP-1s, Bydureon BCise and Byetta, came off the preferred drug list on January 1, 2026. Members taking either are directed to switch to one of the covered alternatives above.3BCBSM Provider Info. Drug Coverage Changes Effective January 1, 2026
How Prior Authorization Works
Since August 2023, every GLP-1 prescribed for diabetes on a BCBSM or BCN commercial plan requires prior authorization. In practice, most prescriptions clear without any paperwork. At the pharmacy counter, the system checks whether the member has a Type 2 diabetes diagnosis code in medical claims from the previous twelve months or a recent pharmacy claim for another diabetes medication. When nothing qualifying is found, the prescriber submits a short attestation confirming Type 2 diabetes. BCBSM does not require clinical documentation at this step; the office simply verifies the diagnosis. Prediabetes and obesity do not qualify under the diabetes pathway.4MCT2D.org. GLP-1s Prescribed for Weight Loss
BCBSM ended coverage of Wegovy (semaglutide), Zepbound (tirzepatide), and Saxenda (liraglutide) for weight loss on fully insured large group commercial plans. The termination took effect January 1, 2025, or on the employer group’s 2025 renewal date if that came later in the year. On January 1, 2026, Saxenda was pulled from the clinical, custom, and preferred drug lists entirely. For members whose plans still include weight loss drug coverage, the covered options are phentermine, phentermine/topiramate ER, Wegovy, and Zepbound. Your Plan Type Decides the Answer
The weight loss exclusion does not sweep every BCBSM member the same way. What kind of plan you have matters.
Fully Insured Large Group Commercial

Self-Funded (ASO) Employer Plans
Self-funded employers that use BCBSM only to administer benefits can still choose to cover GLP-1 weight loss drugs. The 2024 provider alert stated that these groups may keep their existing prior authorization criteria and are not subject to the mandatory termination. MESSA
The Michigan Education Special Services Association, which covers many school employees through BCBSM, was initially outside the termination. As of October 2025, MESSA’s own policy states that it does not cover Saxenda, Wegovy, or Zepbound for weight loss or for other diagnoses such as sleep apnea. Ozempic, Mounjaro, Rybelsus, and Trulicity remain covered for diabetes.8MESSA. Weight Loss
Small Group and Individual Plans
BCBSM’s provider documentation names “fully insured large group commercial members” as the group affected and does not address small group plans separately. Small employer plans and individual marketplace plans generally follow standardized BCBSM formularies without employer-level customization, so members on those plans usually cannot override a formulary exclusion.2BCBSM Provider Info. Medicare Advantage and Part D
BCBSM’s Medicare plans cover GLP-1s for Type 2 diabetes, consistent with federal Medicare rules. Medicare does not generally cover GLP-1s for weight loss. A federal demonstration called the Medicare GLP-1 Bridge is scheduled to run from July 1, 2026, through December 31, 2027, providing a separate pathway for beneficiaries seeking GLP-1s for weight management. Michigan Medicaid
Michigan Medicaid follows its own rules. Under Public Act 22 of 2025, effective January 1, 2026, Saxenda, Wegovy, and Zepbound require prior authorization when prescribed for obesity. Coverage is limited to patients classified as morbidly obese, requires documented failure of other clinically appropriate weight loss interventions including preferred anti-obesity agents, and is framed as a step to prevent the need for higher-cost bariatric surgery. Saxenda and Wegovy moved to non-preferred status, raising copayments from $1 to $3 for affected beneficiaries. Zepbound and Mounjaro Are Not Interchangeable for Coverage
Both drugs contain tirzepatide, but they carry different FDA-approved indications and sit in different coverage categories at BCBSM. Mounjaro is approved for Type 2 diabetes and remains covered. Zepbound is approved for weight management and is excluded on the affected plans alongside Wegovy and Saxenda. BCBSM’s 2024 transitional criteria stated plainly that Zepbound was “not covered for members with Type 2 diabetes,” so a prescriber cannot simply switch products to route around the exclusion.7BCBSM. What Happens on Appeal
Members who have been denied GLP-1 weight loss coverage can request external review through the Michigan Department of Insurance and Financial Services. Recent outcomes show a clear pattern: the plan documents drive the decision.
In File No. 234270-001, decided April 1, 2025, the DIFS Director reversed a Wegovy denial because BCBSM’s own Saver Rx drug list, updated as recently as February 7, 2025, still listed Wegovy as a “nonpreferred drug” rather than an excluded one. The member’s contract required coverage of drugs on the formulary, so the denial was inconsistent with the plan terms and BCBSM was ordered to provide coverage.11Michigan DIFS. In File No. 235707-001-SF (June 23, 2025), a self-funded plan member won reinstatement of Zepbound after BCBSM denied continuation because the patient’s BMI had dropped below 35 during treatment. An independent review organization found the initial BMI of 38.4 established medical necessity and that current BMI alone should not end ongoing therapy.12Michigan DIFS. BCBSM File No. 235707-001-SF
When plan language clearly excludes GLP-1 weight loss drugs, denials have been upheld. In File No. 234317-001 (May 13, 2025), a request for Wegovy to treat obesity and binge eating disorder was denied because the drug list expressly excluded Wegovy.13Michigan DIFS. BCBSM File No. 234317-001 In File No. 239833-001 (October 13, 2025), a member who had lost 67 pounds on GLP-1 therapy was denied continued Zepbound coverage because the benefit certificate contained an express GLP-1 weight loss exclusion effective at the employer’s 2025 renewal.14Michigan DIFS. Before filing an external review, pull your benefit certificate and your plan’s current drug list. If either document still lists the drug as covered, even as nonpreferred, that inconsistency is the argument that has worked.
Why BCBSM Made the Change
BCBSM cited both cost and clinical data. GLP-1 claims hit $1.1 billion in 2024, a 29 percent jump over 2023, and overall pharmacy claims ran nearly 15 percent higher year over year. Between 2022 and 2023, GLP-1 costs alone grew by more than $350 million. BCBSM noted that all claims it pays, averaging $100 million a day, factor into future premiums.1BCBSM MiBluedaily. The insurer also pointed to a Blue Health Intelligence study finding that 58 percent of patients taking GLP-1s for weight loss discontinued use before achieving a clinical benefit, with more than 30 percent dropping out after just four weeks. BCBSM said the exclusion was intended to preserve access for patients with Type 2 diabetes who depend on these medications and that more research is needed on whether GLP-1 weight loss treatment reduces medical costs over time.1BCBSM MiBluedaily.
