Blue Cross and Blue Shield of Texas does cover GLP-1 medications, but the answer depends heavily on why the drug is prescribed and which plan you have. For type 2 diabetes, most BCBSTX plans cover GLP-1s once prior authorization is approved. For weight loss, most plans do not cover them: that benefit is optional and only applies if your employer chose to add it. Members without that election pay full price out of pocket.
Weight Loss Coverage Depends on Your Employer
BCBSTX states on its website that “most plans don’t cover GLP-1s for weight loss.”1BCBSTX. When an employer opts in, the drugs are subject to utilization management, and BCBSTX pairs the benefit with metabolic health programs.2BCBSTX. Diabetes Coverage With Prior Authorization
GLP-1s prescribed for type 2 diabetes are more broadly covered under BCBSTX plans, but prior authorization is required to confirm the diagnosis. Since June 2023, BCBSTX has used integrated medical and pharmacy claims data to automate some approvals: members with a documented type 2 diabetes diagnosis and a claims history for diabetes medications may have prior authorization satisfied automatically.4BCBSTX. When the diagnosis isn’t already in the claims data, the prescribing physician has to send chart notes or lab results. Since November 2023, prescriber attestation alone is not accepted; the actual chart documentation is required.4BCBSTX. - An A1C of 6.5% or higher
- A fasting plasma glucose of 126 mg/dL or higher
- A two-hour oral glucose tolerance test result of 200 mg/dL or higher

Older or less-preferred agents carry step therapy requirements. For drugs like Byetta, exenatide, liraglutide, or Victoza, the provider must document inadequate response, intolerance, or contraindication to semaglutide (Ozempic or Rybelsus), dulaglutide (Trulicity), or tirzepatide (Mounjaro).5MyPrime. Which GLP-1 Drugs Are on the Formulary
As of May 2026, BCBSTX drug lists for self-funded groups include several GLP-1 medications, but availability tracks with whether the employer elected weight management coverage.
- Wegovy (semaglutide injection), including Wegovy HD, is covered for groups that have elected GLP-1 coverage for weight management, subject to utilization management.
- Zepbound (tirzepatide auto-injection) is covered for groups that elect GLP-1 coverage.
- Zepbound KwikPen is excluded from BCBSTX coverage entirely and is available only through Eli Lilly’s direct-to-consumer channel.
- Ozempic tablets in 1.5 mg, 4 mg, and 9 mg strengths have been available since May 4, 2026, for type 2 diabetes, replacing the older Rybelsus formulation. Rybelsus is expected to be phased out by the end of 2026.

Supply Limits Your Plan May Apply
Some BCBSTX employer plans have added optional pharmacy programs that restrict GLP-1 fills. The “GLP-1 New to Therapy” program, available to large and self-funded groups since April 2024, limits initial fills to a 30-day supply for members who haven’t filled a GLP-1 prescription in the previous 120 days. After the initial period, members may become eligible for up to a 90-day supply depending on the plan. Two additional programs — “GLP-1 30-Day Max” and “Anti-Obesity 30-Day Max” — became available in September 2024 and cap all fills at a 30-day supply for both oral and injectable medications. Drugs potentially affected include Mounjaro, Ozempic, Orlistat, Phentermine HCL, Qsymia, Saxenda, Trulicity, Wegovy, and Zepbound, though the specific list depends on what the employer selected.8BCBSTX. What You Pay Without Coverage
When a plan doesn’t cover a GLP-1 for weight loss, the cost falls entirely on the patient. Wegovy’s list price is roughly $1,349 for a 28-day supply of the injectable, though Novo Nordisk offers a self-pay introductory price of $199 per month for the lowest starting doses. Zepbound’s list price for a 28-day supply ranges from $499 to $1,086 depending on the dose, and without insurance or discounts the annual cost can exceed $14,000. Members who have commercial insurance coverage and use manufacturer savings cards can pay as little as $25 per month for Zepbound or Wegovy.9FindHonestCare. Wegovy Cost These savings cards are not available to anyone on a government-sponsored plan such as Medicare or Medicaid.
Texas State Employees and Medicare Members
If you’re covered under HealthSelect of Texas as a state employee, the prescription drug program administered by Express Scripts explicitly excludes drugs prescribed for the treatment of obesity.12Express Scripts. Medicare beneficiaries face different rules. Federal law has historically barred Medicare Part D from covering drugs prescribed for weight loss. CMS launched the “Medicare GLP-1 Bridge,” a temporary demonstration running from July 1, 2026 through the end of 2027, under which eligible beneficiaries can access Wegovy or Zepbound for weight reduction at a fixed $50 copayment per month.14CMS.
