Why Standard Plans Exclude Wegovy

This is a benefit exclusion, not a prior authorization step. That distinction matters. A prior authorization can be satisfied with the right paperwork; a benefit exclusion means the drug is not a covered service under the plan at all. The Self-Funded Employer Exception: The Vida GLP-1 Program

Effective January 1, 2025, Blue Cross NC launched a “GLP-1 for Weight Management” program powered by Vida Health. It is available only to Administrative Services Only (ASO) plans, meaning self-funded employer plans that use Blue Cross NC to administer benefits while the employer decides what to cover.4Blue Cross NC. Clinical criteria track the FDA’s indication for anti-obesity medication:

  • BMI of 30 or above without weight-related comorbidities, or
  • BMI of 27 or above with weight-related comorbidities such as high blood pressure, high cholesterol, or type 2 diabetes.

Members must also be enrolled in a Blue Cross NC pharmacy plan. The program pairs the medication with a registered-dietitian-led coaching component built on cognitive behavioral therapy principles. If You Have Blue Cross NC Medicare Advantage

Standard Medicare Part D does not cover weight-loss drugs, and Blue Cross NC’s 2026 Medicare Advantage formulary does not list Wegovy.5Blue Cross NC. Eligibility depends on BMI and specific conditions:

  • BMI of 35 or higher: no additional diagnosis required.
  • BMI of 30 or higher: with heart failure with preserved ejection fraction, uncontrolled hypertension, or chronic kidney disease (stage 3a or above).
  • BMI of 27 or higher: with pre-diabetes, a previous heart attack, a previous stroke, or symptomatic peripheral artery disease.

The copay is a flat $50 for a 30-day supply. It does not count toward the Part D deductible or out-of-pocket maximum, and Extra Help subsidies cannot reduce it.7Medicare.gov. A Note for State Employees and Retirees

The North Carolina State Health Plan is not Blue Cross NC commercial insurance, even though Blue Cross NC administers it. The State Health Plan stopped covering Wegovy and other GLP-1 weight-loss drugs effective April 1, 2024, after the Board of Trustees voted 4 to 3 to end the benefit. Confirm What Your Specific Plan Says

Coverage varies plan by plan. An employer-sponsored plan may have opted into the Vida program or written weight-loss drugs into its own benefits. The first step is a call to member services using the number on your ID card, asking specifically whether Wegovy is covered under your plan for weight management.

File an Appeal

If your plan has any pathway to coverage and your claim was still denied, you or your doctor can appeal. Start by reviewing the Explanation of Benefits to identify the exact reason for the denial, then submit a written appeal within six months of the denial notice. Useful documentation includes your current BMI, records of prior weight-loss treatments that did not work, any weight-related conditions, and a medical necessity letter from your provider. Paying Out of Pocket

Several routes bring the cost below the roughly $1,350 list price:

  • Manufacturer copay card. Patients with commercial insurance, even insurance that doesn’t cover Wegovy, may pay as little as $25 per month, with a maximum savings of $100 per month. Members of government plans are not eligible.13Wegovy. What To Pay for Wegovy
  • Self-pay pricing through NovoCare. The 1.5 mg and 4 mg doses run $149 per month through August 31, 2026 (the 4 mg dose then rises to $199).
