Most Blue Cross and Blue Shield of Texas plans do not cover weight loss drugs. What that means in practice: coverage exists only where the plan sponsor has specifically chosen to include it. A drug that appears on one employer’s BCBSTX formulary may be excluded from another’s. The important thing is not what BCBSTX covers “in general” but what your plan document says.
Employer-Sponsored Plans: An Optional Add-On

BCBSTX’s consumer FAQ identifies Wegovy, Saxenda, and Zepbound as the FDA-approved GLP-1s for weight loss.1BCBSTX. Prior Authorization: What You Have To Qualify For
Even when your plan covers weight loss drugs, you’ll almost certainly need prior authorization before starting treatment.1BCBSTX. GLP-1s for Weight Loss The clinical criteria used by Prime Therapeutics for BCBSTX’s weight management PA program (effective April 2026) require:
- A BMI of 30 kg/m² or higher, or a BMI of 27 kg/m² or higher with at least one weight-related condition such as type 2 diabetes, high blood pressure, high cholesterol, cardiovascular disease, or obstructive sleep apnea. - Lifestyle modification alongside the drug: a reduced-calorie diet and physical activity. Patients who have never participated in a comprehensive lifestyle program supervised by a trained professional are encouraged to complete one before starting medication.6Prime Therapeutics. - Ongoing monitoring: providers assess efficacy monthly for the first three months, then every three months. If a patient hasn’t lost at least 5% of starting body weight after 12 weeks, the provider should reconsider whether to continue the medication.6Prime Therapeutics. Texas Public-Employee Plans That Exclude Weight Loss Drugs
If you’re covered under one of Texas’s large public-employee plans administered through the BCBSTX network, the answer is a firm no.
HealthSelect of Texas, which covers state employees and retirees, explicitly excludes “drugs prescribed for the treatment of obesity” in its master benefit document.7Express Scripts. HealthSelect of Texas Prescription Drug Program
TRS-Care, the health plan for Texas teacher retirees, excludes drugs with an FDA indication for weight loss, weight reduction, or dietary control, naming Saxenda and Wegovy as examples. The exclusion applies even when a participant has medical conditions that could benefit from weight loss and even when a physician has written the prescription. TRS does cover bariatric surgery for members on its Primary and Primary+ plans who meet qualification criteria, and offers wellness benefits at no extra cost, including obesity counseling, nutrition counseling, one-on-one health coaching, and discounted gym memberships.8Teacher Retirement System of Texas. Because coverage varies so much across BCBSTX plans, the only reliable way to know what your plan covers is to check directly. A few ways to do that:
