Does BCBSTX Cover Weight Loss Drugs? Plans, GLP-1s, and Costs

Most Blue Cross and Blue Shield of Texas plans do not cover weight loss drugs. Whether BCBSTX covers weight loss drugs for you depends almost entirely on the plan you’re enrolled in: for employer-sponsored coverage, the employer has to have opted into a weight management drug benefit, and for some large public-employee plans in Texas, weight loss drugs are excluded no matter what. If your doctor prescribes a GLP-1 for weight loss and your plan doesn’t cover it, you’ll typically pay the full cost yourself.¹

Why Coverage Depends on Your Specific Plan

BCBSTX has no single, blanket policy for weight loss drugs. The insurer’s own FAQ states that “most plans don’t cover GLP-1s for weight loss.”¹ Its standard drug lists note that “some plans may exclude coverage for certain agents or drug categories, like those used for erectile dysfunction or weight loss.”²

What that means in practice: coverage exists only where the plan sponsor has specifically chosen to include it. A drug that appears on one employer’s BCBSTX formulary may be excluded from another’s. The important thing is not what BCBSTX covers “in general” but what your plan document says.

Employer-Sponsored Plans: An Optional Add-On

For self-funded employer groups (Administrative Services Only, or ASO), BCBSTX treats weight loss medication as a “custom benefit option.” Employers can elect to add it, and those that do get a dedicated prior authorization program for chronic weight management drugs, run separately from GLP-1 coverage for type 2 diabetes.³

If your employer hasn’t opted in, the drugs aren’t covered, even if you’ve seen them on a BCBSTX drug list somewhere else. Verify before you fill a prescription. Your HR or benefits administrator can confirm whether weight management drug coverage is part of your plan.

Which GLP-1s Are Covered When an Employer Opts In

For self-funded groups that elect GLP-1 weight management coverage, a BCBSTX producer communication dated May 2026 lays out the current picture:

Wegovy (semaglutide injection) is covered, including the newer Wegovy HD formulation.⁴
Zepbound auto-injection (tirzepatide) is covered.⁴
Zepbound Kwikpen is excluded because it is available only through a direct-to-consumer pathway.⁴
Foundayo (orforglipron calcium tablets) is under clinical review for drug list placement after FDA approval; coverage exception requests are available in the meantime for members whose groups provide weight management coverage.⁴
Saxenda (liraglutide) is listed among applicable drugs in BCBSTX’s weight management programs.⁵

BCBSTX’s consumer FAQ identifies Wegovy, Saxenda, and Zepbound as the FDA-approved GLP-1s for weight loss.¹ Employers that opt in may also apply supply-management rules that limit initial fills to a 30-day supply, covering drugs such as Wegovy, Zepbound, Saxenda, Ozempic, Mounjaro, Trulicity, Qsymia, Phentermine, and Orlistat depending on the employer’s selections.⁵

Prior Authorization: What You Have To Qualify For

Even when your plan covers weight loss drugs, you’ll almost certainly need prior authorization before starting treatment.¹ The clinical criteria used by Prime Therapeutics for BCBSTX’s weight management PA program (effective April 2026) require:

A BMI of 30 kg/m² or higher, or a BMI of 27 kg/m² or higher with at least one weight-related condition such as type 2 diabetes, high blood pressure, high cholesterol, cardiovascular disease, or obstructive sleep apnea. Lower thresholds (BMI 23 kg/m² or higher) apply for South Asian, Southeast Asian, and East Asian populations.⁶
Lifestyle modification alongside the drug: a reduced-calorie diet and physical activity. Patients who have never participated in a comprehensive lifestyle program supervised by a trained professional are encouraged to complete one before starting medication.⁶
Ongoing monitoring: providers assess efficacy monthly for the first three months, then every three months. If a patient hasn’t lost at least 5% of starting body weight after 12 weeks, the provider should reconsider whether to continue the medication.⁶

For children age two and older, the criteria require a BMI at or above the 95th percentile (or 30 kg/m², whichever is lower) for obesity, or at or above the 85th percentile with a related comorbidity.⁶

Texas Public-Employee Plans That Exclude Weight Loss Drugs

If you’re covered under one of Texas’s large public-employee plans administered through the BCBSTX network, the answer is a firm no.

HealthSelect of Texas, which covers state employees and retirees, explicitly excludes “drugs prescribed for the treatment of obesity” in its master benefit document.⁷

TRS-Care, the health plan for Texas teacher retirees, excludes drugs with an FDA indication for weight loss, weight reduction, or dietary control, naming Saxenda and Wegovy as examples. The exclusion applies even when a participant has medical conditions that could benefit from weight loss and even when a physician has written the prescription. TRS does cover bariatric surgery for members on its Primary and Primary+ plans who meet qualification criteria, and offers wellness benefits at no extra cost, including obesity counseling, nutrition counseling, one-on-one health coaching, and discounted gym memberships.⁸ In January 2025, TRS issued a request for information exploring an optional standalone weight loss program that participants could purchase as a buy-up, including GLP-1 access, lab work, provider visits, and nutrition counseling.⁹

How To Check Your Own Coverage

Because coverage varies so much across BCBSTX plans, the only reliable way to know what your plan covers is to check directly. A few ways to do that:

Log in to Blue Access for Members at mybam.bcbstx.com and use the Drug Search feature. It will show whether a specific medication is covered under your plan and what your cost share would be.¹⁰
If you know your drug list name, review it on the BCBSTX drug lists page without logging in. If you don’t know your list name, log into your account first.¹⁰
Use MyPrime.com if your pharmacy benefits are managed by Prime Therapeutics; the site offers 24/7 access to drug coverage information.¹⁰
Ask your pharmacist to check your insurance information on file for coverage and cost.¹¹
Call the number on your member ID card for coverage and prior authorization questions.

If a drug isn’t covered or costs too much, BCBSTX suggests asking your doctor or pharmacist about less expensive covered alternatives or generics.¹¹

What GLP-1s Cost Without Coverage

If your plan doesn’t cover weight loss drugs, out-of-pocket costs are high but no longer at list price for everyone. List prices generally run over $1,000 per month. Novo Nordisk (maker of Wegovy) and Eli Lilly (maker of Zepbound) both offer direct-purchase programs at reduced prices: roughly $499 per month for either drug, with Zepbound’s starter dose available at $349 per month.¹² Manufacturer savings programs accessible through platforms like GoodRx can bring Wegovy costs down further, to roughly $199 to $349 per month.¹³
