Does MassHealth Cover GLP-1? Drugs, Conditions, and Alternatives

MassHealth covers GLP-1 medications when they are prescribed for type 2 diabetes or prediabetes, and that coverage continues without change. Starting July 3, 2026, MassHealth will no longer cover GLP-1s or any other drug prescribed solely for weight loss. Members with certain other qualifying medical conditions, such as established cardiovascular disease, moderate-to-severe sleep apnea, or advanced fatty liver disease, can still get coverage through a prior authorization process.1Mass.gov. The rule is codified at 130 CMR 406.413(B) and applies across all MassHealth plan types, including fee-for-service, managed care organizations, One Care, and Senior Care Options. Any drug used off-label for weight loss is also non-payable after July 3, 2026.1Mass.gov. Which GLP-1 Drugs MassHealth Still Covers

The coverage split is driven by what a drug is prescribed for, not by the drug itself. Some GLP-1 molecules have separate brand names for their diabetes and weight-loss versions.

Covered for type 2 diabetes and prediabetes, unaffected by the change:

  • Ozempic (semaglutide injection)
  • Mounjaro (tirzepatide)
  • Trulicity (dulaglutide)
  • Rybelsus (semaglutide tablets)

Not covered when prescribed for weight loss after July 3, 2026:

  • Wegovy (semaglutide)
  • Zepbound (tirzepatide)
  • Saxenda (liraglutide)
  • Other anti-obesity medications: benzphetamine, diethylpropion, orlistat (Xenical), phendimetrazine, phentermine (Adipex-P, Lomaira), and phentermine/topiramate extended-release

If your GLP-1 was prescribed for type 2 diabetes or prediabetes, MassHealth is directing prescribers to switch you to a diabetes-indicated GLP-1 under existing antidiabetic drug criteria rather than routing you through the anti-obesity authorization pathway.4Point32Health. Medical Conditions That Still Qualify for Coverage

Even after the weight-loss indication is eliminated, MassHealth will continue to authorize GLP-1s for members with specific conditions beyond obesity. A prescriber must submit a prior authorization request with documentation, and approvals last up to six months.5Mass.gov. MassHealth Pharmacy Bulletin, Issue 5, March 2026

The qualifying conditions are:

  • Cardiovascular disease. Reduction of major adverse cardiovascular events in adults with established heart disease (history of heart attack, stroke, symptomatic peripheral arterial disease, or revascularization) and a BMI above 27 kg/m².1Mass.gov. MassHealth Pharmacy Bulletin, Issue 7, May 2026
  • Obstructive sleep apnea. Moderate to severe OSA in adults with a BMI above 30 kg/m². Documentation must include the member’s apnea-hypopnea index score.5Mass.gov. MassHealth Pharmacy Bulletin, Issue 5, March 2026
  • MASH (metabolic dysfunction-associated steatohepatitis). Non-cirrhotic MASH with moderate to advanced fibrosis at stage F2–F3.   • Members under 21. All requests for pediatric members are reviewed for medical necessity under the federal Early and Periodic Screening, Diagnostic, and Treatment requirement, which provides a protected pathway regardless of the broader coverage exclusion.4Point32Health. MassHealth has acknowledged that some members currently stable on a weight-loss GLP-1 “may no longer be eligible under these revised criteria and will be denied coverage at the time of recertification.” Recertification uses the member’s baseline BMI from before they started any GLP-1, not their current weight.6MassHealth Drug List. What Happens If You Have a Current Prescription

There is no grandfathering. All prior authorizations submitted before February 17, 2026 will be end-dated for July 2, 2026, regardless of the original indication. To continue coverage past that date, your prescriber must submit a new prior authorization documenting one of the qualifying conditions above. Wegovy also becomes the sole preferred GLP-1 for the remaining covered indications as of July 3, 2026, and Zepbound loses its preferred status. Members with an approved Zepbound prior authorization will have it automatically replaced with one for Wegovy for the rest of the original authorization period, and a new prescription must be submitted. Options If You Lose Coverage

Start with your prescriber. Ask whether your medical history supports one of the qualifying indications, particularly cardiovascular disease, sleep apnea, or MASH. If any applies, a new prior authorization with the required documentation may keep you covered.5Mass.gov. Paying out of pocket is expensive. Without insurance, these drugs typically cost around $450 per month.2WBUR. Mass Cutting GLP-1 Coverage Medicaid Wegovy Zepbound Novo Nordisk offers a self-pay savings program for Wegovy, with injectable pricing that runs from roughly $199 per month at lower starting doses to $349–$399 per month at maintenance doses. A tablet formulation runs from about $149 to $299 per month depending on dose. That savings program explicitly excludes government insurance beneficiaries, so it would only apply after a member is no longer enrolled in MassHealth or any other government program.7Novo Nordisk. Why MassHealth Is Making This Change

Cost is the stated reason. In 2025, prescription drug spending at MassHealth rose 18 percent and made up more than a quarter of total per-member cost growth, with GLP-1s responsible for nearly half of that drug spending increase.10Mass.gov. Under federal Medicaid law, states are not required to cover drugs used for weight loss. A long-standing statutory exception in the Medicaid Drug Rebate Program lets states exclude these medications from their formularies, and most already do.
