Medi-Cal does cover braces in California, but only for enrollees under 21 and only when the orthodontic problem is severe enough to be medically necessary. Cosmetic straightening and mild crowding do not qualify. To determine medical necessity, the program uses a scoring tool called the Handicapping Labio-Lingual Deviation (HLD) Index California Modification, and a child needs either a score of at least 26 points or one of several automatically qualifying conditions to be approved.1Medi-Cal Dental. Who Qualifies
Two things have to be true. The child must be enrolled in Medi-Cal, and the orthodontic condition must meet the medical-necessity standard. Eligibility ends the day the enrollee turns 21, with no extensions.1Medi-Cal Dental. California Medi-Cal Dental Orthodontic Seminar Packet The reason coverage exists at all for minors is the federal Early and Periodic Screening, Diagnostic and Treatment (EPSDT) benefit, which requires state Medicaid programs to pay for medically necessary services for children, including orthodontic care.2Medicaid.gov. Adults enrolled in Medi-Cal generally do not get orthodontic coverage. California law lists orthodontic services among the exclusions from adult dental benefits.3California Legislative Information. California Welfare and Institutions Code 14132 The one narrow exception is orthodontic work for cleft palate deformities, which is handled through California Children’s Services, a separate program.
How Medical Necessity Is Measured
An orthodontist examines the child, takes X-rays and diagnostic casts (plaster or digital models of the teeth), and scores the relevant conditions on the HLD sheet. Twenty-six points is the threshold.4California Department of Social Services. Orthodontic Services HLD Index Requirements
Some conditions skip the scoring entirely and qualify on their own. If any of the following is present and documented, no point total is needed:5Medi-Cal Dental. Provider Bulletin Volume 35 Number 11
- Cleft palate deformity, documented with casts, intraoral photos, or a specialist report.
- Craniofacial anomaly, documented by a credentialed specialist.
- Deep impinging overbite in which the lower front teeth are destroying palatal soft tissue, with visible damage or attachment loss.
- Anterior crossbite in which a front tooth crosses behind the lower teeth and is causing gum recession and attachment loss.
- Severe traumatic deviation, such as loss of a permanent front tooth from trauma, ankylosis of a permanent tooth, or other significant pathology.
- Overjet greater than 9mm with incompetent lips, or a reverse overjet (underbite) greater than 3.5mm causing chewing and speech difficulty.
If the case doesn’t have an automatic qualifier, the other conditions on the HLD sheet are scored and have to add up to 26 or more. A child who falls just short can still be approved under EPSDT if the orthodontist submits a written narrative showing the treatment will correct or improve the condition. It’s uncommon but worth knowing if the score comes in close.
What Coverage Includes
Approved cases receive traditional metal braces.6IEHP. Medi-Cal Dental Coverage Because the program is aimed at correcting function rather than appearance, clear aligners and ceramic brackets are not standard covered options. What is included: the diagnostic workup (X-rays, cephalometric films, diagnostic casts), periodic adjustment visits during treatment, and retainers afterward.
The orthodontist has to be enrolled as a Medi-Cal Dental provider. Not every orthodontist participates, so confirm enrollment before the initial evaluation. Medi-Cal Dental publishes a provider search tool for locating participating orthodontists.
How Approval Works
Every orthodontic case needs prior authorization from Medi-Cal Dental before treatment starts.7California Department of Health Care Services. - The completed HLD Index California Modification score sheet.
- A full-mouth X-ray series or panoramic film, plus a cephalometric head film with tracings.
- Diagnostic casts, trimmed and labeled with the patient’s name and ID, mailed separately from the TAR.
Medi-Cal Dental reviews the records, confirms the score or qualifying condition, and approves or denies the request. Craniofacial anomaly cases skip the casts but need specialist documentation.
If the Request Is Denied
A denial isn’t the end of the process. Medi-Cal sends a Notice of Action (NOA) explaining the reason. You have 60 calendar days to file an appeal, and either you or the orthodontist can submit it in writing or by phone.
Standard appeals get a written decision within 30 calendar days. Expedited appeals, reserved for urgent health concerns, are decided within 72 hours. If your child is already receiving services and you want them to continue during the appeal, you have to request “aid paid pending” within 10 days of the NOA.
If the appeal is denied, the next step is a state fair hearing through the California Department of Social Services, which must be requested within 120 calendar days of the appeal decision. You can also apply for an Independent Medical Review through the Department of Managed Health Care within 180 days, though that route usually requires finishing the plan’s own appeal process first unless there’s an immediate health concern.
What Braces Cost Without Coverage
If your child doesn’t qualify, the out-of-pocket price for traditional metal braces in California generally runs $3,000 to $7,000 depending on case complexity and location. An initial orthodontic consultation with X-rays and study models is typically about $100 to $150 on its own. Those numbers are worth keeping in mind when weighing whether to pursue an evaluation and, if the score is borderline, an EPSDT narrative or an appeal.
