New York Medicaid does not cover Wegovy when it is prescribed for weight loss. It can be covered, under strict conditions, when it is prescribed to reduce the risk of serious cardiovascular events in adults who already have heart disease. Everything below explains where that line sits and what you can do on either side of it.
Why Weight Loss Prescriptions Are Not Covered
NYRx, the centralized pharmacy program that handles the Medicaid drug benefit in New York, states plainly that “weight loss has never been a Medicaid-approved reason for covering a drug,” and names Wegovy, Ozempic, Qsymia, and Mounjaro as drugs that are not covered when prescribed for that purpose.1NYRx. Two laws sit behind this. Federal law, at Section 1927(d)(2) of the Social Security Act, lets state Medicaid programs refuse to cover drugs used for “anorexia, weight loss, or weight gain.”2KFF. A note on Medicaid managed care plans, because people often assume a plan might cover something the state does not. It cannot, at least not for this. Mainstream Medicaid managed care members in New York get their pharmacy benefit through NYRx directly, on a single statewide formulary, and plans have no authority to add weight loss drugs on their own.5New York State Department of Health. The Cardiovascular Risk Exception
Wegovy has a second FDA-approved use that is not “weight loss,” and that changes the analysis. In March 2024, the FDA approved semaglutide (Wegovy) to reduce the risk of cardiovascular death, heart attack, and stroke in adults with established cardiovascular disease who are obese or overweight. In the trial supporting approval, major cardiovascular events occurred in 6.5% of the Wegovy group versus 8% in the placebo group.6U.S. Food and Drug Administration. Because that indication falls outside the federal weight-loss carve-out, states generally must cover it. New York’s Drug Utilization Review Board deferred the question in October 2024 and then, on February 27, 2025, unanimously approved prior authorization criteria for Wegovy prescribed to reduce cardiovascular risk.7New York State Department of Health. The criteria are narrow. For an initial authorization, a patient must:
- Have established cardiovascular disease, documented as a prior heart attack, stroke, or peripheral arterial disease.
- Have a BMI of 40 or higher.
- Not have type 1 or type 2 diabetes. Diabetic patients are directed to other covered GLP-1 drugs.
- Already be taking prescribed cardiovascular medications, such as blood pressure, cholesterol, or anti-clotting drugs, for at least six months.
- Have participated in a reduced-calorie diet, increased physical activity, and behavioral modifications for at least six months before starting Wegovy.
Initial authorization runs up to six months. Renewals can extend up to 12 months. The program allows two treatment attempts per lifetime. The Department of Health has said it plans to use these criteria as a template for other GLP-1 drugs prescribed outside of diabetes.7New York State Department of Health. What NY Medicaid Does Cover for Related Conditions
If you were hoping Wegovy would be your route to a covered GLP-1, know that NYRx does cover several GLP-1 receptor agonists for type 2 diabetes. The preferred drug list includes Ozempic, Trulicity, Victoza, and exenatide, each subject to clinical criteria.8NYRx. Preferred Drug Quick List Mounjaro is covered for diabetes as well.1NYRx. Non-preferred GLP-1 drugs need prior authorization, and switching drugs or doses while more than 25% of a current fill remains also triggers prior authorization.9NYRx. For obesity itself, covered options exist that are not drugs. Bariatric surgery, including gastric bypass, sleeve gastrectomy, and adjustable gastric banding, is covered with prior authorization for both managed care and fee-for-service members.10George Washington University. Medicaid Obesity Coverage – New York Hospital-based weight management programs, such as the one at NYC Health + Hospitals/Bellevue, offer medical weight management, intensive nutritional treatment, and behavioral counseling that are generally covered by Medicaid and Medicaid managed care subject to medical necessity.11NYC Health + Hospitals. Bariatric Surgery and Weight Management Standalone adult obesity-specific behavioral counseling is not explicitly defined in state policy; providers are expected to address nutrition and physical fitness during regular office visits.
Paying Out of Pocket If You Are Not Covered
If Wegovy is not covered for you, the manufacturer discounts most patients rely on are closed off. Novo Nordisk’s Wegovy savings card explicitly excludes anyone receiving benefits from a government-funded health program, including Medicaid.12NovoCare. The list price for the injectable form is $1,349.14Medical News Today. Wegovy Cost An oral version of Wegovy, approved by the FDA in December 2025 and launched in January 2026, is available at a starting-dose price of $149 per month for self-pay patients.15CNN. Wegovy Pill FDA Approval A federal pricing program called TrumpRx lists the Wegovy pill at $149 per month and the pen at $199 per month, though whether individual Medicaid enrollees paying out of pocket can access those prices, as opposed to states buying at negotiated rates, is not clear from public information.16TrumpRx. Proposals That Could Change the Answer
Two bills in the New York 2025-2026 legislative session would require Medicaid to cover anti-obesity drugs. Senate Bill S5798, sponsored by Senator Luis R. Sepúlveda, would amend the Social Services Law to cover FDA-approved chronic weight management drugs for adults with obesity who have at least one weight-related condition.17New York State Senate. Neither has advanced. S5798 was referred to the Senate Health Committee in March 2025, re-referred in January 2026, and has had no hearings or votes.19New York State Assembly. At the federal level, CMS launched a voluntary five-year initiative called the BALANCE model in December 2025, under which CMS negotiates pricing and coverage terms for GLP-1 drugs on behalf of participating state Medicaid programs and Medicare Part D plans. The Medicaid component launched in May 2026, with a state application deadline of July 31, 2026.21Centers for Medicare and Medicaid Services.
