New York Medicaid does not cover Zepbound when it is prescribed for weight loss. The state’s Medicaid pharmacy program, NYRx, treats drugs used for weight loss as a state plan exclusion, and the April 2025 New York State Medicaid Update names Zepbound specifically.1NY State Dept of Health. Medicaid Update April 2025 Bills to change this have been introduced in Albany but none have moved out of committee as of mid-2026.
Why New York Excludes Zepbound
Federal law lets states choose whether to exclude drugs used for “anorexia, weight loss, or weight gain” from Medicaid coverage under Section 1927(d)(2) of the Social Security Act. The January 2025 Medicaid Update repeats the broader rule that drugs used for weight loss are “not reimbursable by NYS Medicaid.”3NY State Dept of Health. Same Drug as Mounjaro, Different Rules
Zepbound and Mounjaro are both tirzepatide, but they carry different FDA approvals. Mounjaro is approved for type 2 diabetes, which is a medically accepted indication states must cover. Zepbound is approved for chronic weight management, which sits inside New York’s optional exclusion.4KFF. Medicaid Coverage of and Spending on GLP-1s
Neither brand appears on New York’s preferred drug list. The NYRx Preferred Drug Quick List, revised June 2026, lists other GLP-1 agonists for diabetes (exenatide, Ozempic, Trulicity, Victoza) but no form of tirzepatide.5FHSC. NYRx Preferred Drug Quick List
The Sleep Apnea Approval and the Wegovy Comparison
Zepbound received FDA approval in December 2024 for moderate to severe obstructive sleep apnea in adults with obesity.6Oregon DURM. Tirzepatide OSA Prior Authorization Update In theory, that gives it a non-weight-loss indication. In practice, NYRx has no prior authorization form or coverage pathway for Zepbound, and available information does not show whether the state plans to build one.
Wegovy shows what a pathway looks like when the state chooses to open one. It has a New York prior authorization route tied to its FDA approval for reducing cardiovascular risk in patients with established heart disease. The PA form requires documentation of cardiovascular disease, six months of adherence to cardiovascular medications, and six months of lifestyle modifications before starting treatment.7FHSC. Legislation That Would Change the Rule
Three bills in the 2025-2026 session would require Medicaid to cover anti-obesity medications. All sit in committee.
- Senate Bill S5798, sponsored by Senator Sepúlveda, would amend the Social Services Law to require Medicaid and its managed care plans to cover FDA-approved chronic weight management drugs for adults with obesity who have at least one weight-related condition. The sponsor’s memo concedes that “New York State Medicaid does not presently cover prescription drugs approved by the FDA for chronic weight loss.” Introduced March 2025 and held in the Senate Health Committee.8New York State Senate. - Assembly Bill A9360, sponsored by Assemblymember Santabarbara, would require Medicaid coverage of GLP-1 receptor agonists for obesity, metabolic disorders including prediabetes and type 2 diabetes, and autism-related compulsive eating behaviors. - Assembly Bill A2715 is broader, covering intensive behavioral therapy, bariatric surgery, and FDA-approved anti-obesity drugs, and it would prohibit more restrictive deductibles or copays for obesity treatment than for other conditions. Paying for Zepbound Without Coverage
Eli Lilly sells Zepbound vials directly to patients through its LillyDirect platform. Self-pay prices as of late 2025 start at $299 per month for the 2.5 mg dose, $399 for 5 mg, and $449 for higher doses. The Zepbound Savings Card is not an option here. Its terms exclude anyone enrolled in Medicaid, Medicare, or any other government-funded healthcare program.12Eli Lilly. Zepbound Full Terms and Conditions
Compounded tirzepatide was widely available during the Zepbound shortage, but those options are expected to become unavailable now that the shortage has resolved.13NPR. Zepbound Eli Lilly Direct Cost
Obesity Treatments New York Medicaid Will Cover
Weight loss medications are excluded, but other obesity treatments are not. NYC Health + Hospitals/Bellevue runs a weight management program that accepts Medicaid and Medicaid managed care plans. Covered services include bariatric surgery (gastric bypass, sleeve gastrectomy, and adjustable gastric banding), medical weight management with lifestyle counseling, intensive nutritional treatment, and psychological counseling. Surgical candidates generally need a BMI above 40, or above 35 with an obesity-related condition such as diabetes or high blood pressure.14NYC Health + Hospitals.
