Who Qualifies for Coverage
A prescription alone is not enough. Your provider has to submit a prior authorization request and document that you meet each of the state’s criteria. For adults 18 and older, the BMI thresholds are:
- A baseline BMI of 30 or higher, with no other condition required, or
- A baseline BMI of 27 or higher with at least one weight-related comorbidity such as hypertension, type 2 diabetes, or dyslipidemia.

Beyond BMI, the prior authorization request must include:
- Your current weight in kilograms.
- Documentation that you have started or are maintaining a reduced-calorie diet (or are working with a registered dietitian) along with a physical activity regimen, unless exercise is medically contraindicated.
- Confirmation that you are not taking another weight-loss medication at the same time.
- Confirmation of no contraindications, or a prescriber note explaining why benefits outweigh risks.

How Renewals Work
After the first six months, continued coverage depends on results. Adults must show at least a 5% reduction in body weight from baseline. Renewals run 12 months at a time. Once you pass the 18-month mark, subsequent renewals require you to show you are maintaining the weight loss achieved during that first approval period. What You Pay
Medical Assistance enrollees pay no copays and no deductibles for prescription drugs, and that includes Wegovy once prior authorization has been approved. Dual-eligible enrollees are the exception. If you have both Medical Assistance and Medicare Part D, your prescriptions go through your Part D plan rather than MA. The Managed Care Wrinkle
Most MHCP enrollees get their coverage through a managed care organization such as UCare, Hennepin Health, Blue Plus, or Medica. Every MCO must follow the state’s Uniform Preferred Drug List, and Wegovy is not on it. Ozempic Is Not a Substitute
Wegovy and Ozempic contain the same active ingredient, semaglutide, but they have different FDA approvals. Ozempic is approved only for type 2 diabetes. Wegovy is approved for chronic weight management. Minnesota Medicaid generally does not cover Ozempic when it is prescribed off-label for weight loss in patients who do not have diabetes. If Your Prior Authorization Is Denied
Denials can be challenged, and the route depends on whether you are in managed care or fee-for-service.
If you are in an MCO, you will get a written denial notice explaining the reason and your rights. You have 90 days to file an internal appeal with the MCO. The MCO must decide within 30 days, or within 72 hours for an expedited appeal when a delay could put your health at risk.5Minnesota Department of Human Services. Fee-for-service enrollees, and MCO enrollees who prefer to skip the plan-level process, can file an internal medical-necessity appeal with Acentra Health, the state’s utilization review contractor, within 180 days of the denial. You can also request a State Fair Hearing through the DHS Appeals Division without first exhausting the internal appeal. A human services judge conducts the hearing, usually by phone, and issues a decision.
