TennCare does not cover gym memberships. Tennessee’s Medicaid program lists no fitness center benefit in any of its benefit packages, and none of the managed care organizations that deliver TennCare coverage advertise gym access as a standard perk. If you have seen a Medicaid-linked gym benefit online, it almost certainly belongs to a Medicare Advantage plan or a Tennessee state employee program, not to TennCare.
Why Gym Memberships Aren’t a TennCare Benefit
TennCare’s benefit packages, updated in April 2026, itemize covered services across physician visits, hospital care, pharmacy, behavioral health, dental, vision, therapy, home health, and emergency transportation. Every covered service has to meet the program’s definition of medically necessary, and gym memberships appear nowhere in packages A through M.1Tennessee.gov. TennCare Benefit Packages
There is a narrow door in the rules. TennCare allows managed care organizations to offer “cost-effective alternative services” at their own discretion, and a TennCare policy manual pre-approves “weight reduction programs for the treatment of obesity” as one such alternative that plans can fund without extra sign-off.2Tennessee Secretary of State. It doesn’t create an entitlement, and no current TennCare MCO uses it to offer a gym membership.
The One Exception: BlueCare Plus
BlueCare Plus, which serves members dually eligible for both Medicaid and Medicare, does include fitness access. Members can use fitness centers locally and nationwide, join group exercise classes, and tap home exercise resources.4BlueCross BlueShield of Tennessee. BlueCare Plus Helpful Tools and Resources The fitness benefit rides on the Medicare Advantage side of the plan. If you have TennCare only, without Medicare, you don’t qualify.
What Each TennCare Plan Offers Instead
TennCare contracts with four managed care organizations: BlueCare, UnitedHealthcare Community Plan, Wellpoint (formerly Amerigroup), and TennCare Select.5Tennessee.gov. TennCare Managed Care Organizations Wellness support across the plans comes as coaching, screening, and education rather than gym time.
- UnitedHealthcare Community Plan covers obesity screening, health coaching, and support for diet and exercise choices. Members also get free health education classes on nutrition and diabetes, plus free transportation to those classes.6UnitedHealthcare. UnitedHealthcare Community Plan – TennCare
- Wellpoint runs a Healthy Rewards program that pays out gift cards when members complete wellness visits and screenings. Weight Management TennCare Does Cover
Since August 2025, TennCare has covered FDA-approved weight-management medications, including GLP-1 receptor agonists such as Wegovy and Zepbound.10American Diabetes Association. American Diabetes Association Celebrates TennCare Decision To get them, you have to be taking part in complementary nutritional and lifestyle changes, including dietary modification, increased physical activity, and a structured behavioral intervention or comprehensive weight management program.11OptumRx Content Hub. TennCare also covers bariatric surgery for adults over eighteen and visits with registered dietitians for members with diabetes or chronic kidney disease. A 2022 state task force recommended expanding dietitian access and mental health counseling for members with an obesity diagnosis.12Tennessee General Assembly. Fitness Programs People Confuse With TennCare
SilverSneakers and Renew Active are Medicare Advantage benefits, not Medicaid benefits. Renew Active runs through select UnitedHealthcare Medicare Advantage and qualifying AARP Medicare Supplement plans, and covers free gym memberships along with on-demand workout videos and brain-health tools.13UnitedHealthcare. The Partners for Health fitness center discount is a Tennessee state employee benefit. State employees on BlueCross BlueShield of Tennessee can access discounted gym memberships from $19 per month with no long-term contract, and employees on Cigna can join Active & Fit Direct for $28 per month after an enrollment fee.15Tennessee.gov. The YMCA of Middle Tennessee offers a 50% joining-fee discount for BlueCross BlueShield of Tennessee customers, but its website lists no program for TennCare or Medicaid members.16YMCA of Middle Tennessee. Insurance-Related Memberships

Benefits can vary by eligibility category and managed care plan, so call the number on the back of your TennCare member ID card to confirm what your specific plan offers. You can also reach TennCare Connect at 855-259-0701.17Tennessee.gov. TennCare Covered Services Ask directly whether your plan offers any cost-effective alternative services tied to fitness or weight management. Those discretionary benefits are handled case by case and won’t necessarily show up on the plan’s public benefits page.
