Yes, TennCare covers weight loss injections for members who meet clinical criteria. Since August 1, 2025, Tennessee’s Medicaid program has covered FDA-approved obesity medications, including the injectables Wegovy, Zepbound, Saxenda, and Imcivree, for adults 21 and older. Which Injections TennCare Covers
TennCare sorts obesity medications into preferred and non-preferred drugs. All the injectable options require prior authorization.
Preferred injections:
- Wegovy (semaglutide), up to 4 injections per month.
- Zepbound (tirzepatide), up to 4 injections per month.
Non-preferred injections:
- Saxenda (liraglutide), up to 3 mg per day.

To get a non-preferred drug approved, the patient generally has to have tried and failed, been intolerant of, or had a contraindication to two preferred agents first.3OptumRx. Who Qualifies
Eligibility depends on age and body mass index, drawn from the prior authorization form revised in April 2026.
- Adults 18 and older: BMI over 30, or BMI over 27 with at least one weight-related condition such as hypertension, high cholesterol, diabetes, coronary heart disease, fatty liver disease, or obstructive sleep apnea.

BMI alone is not enough. The prescribing provider has to attest that you are actively participating in lifestyle changes, meaning some combination of dietary modification, increased physical activity, structured behavioral intervention, or a comprehensive weight management program. Zepbound has a separate approval path for obstructive sleep apnea. To qualify on that basis, you need a BMI over 30, 15 or more respiratory events per hour on a sleep study, and at least three months of unsuccessful CPAP or BiPAP therapy.3OptumRx. GLP-1 Agonists PA Form
How to Get Approved
Prior authorization is initiated by your prescriber, not by you.
- Talk to your doctor about your weight history, BMI, and any related conditions. Your provider will need to document all of it, plus your lifestyle changes.

What You Will Pay
Adult TennCare members pay nominal copays: $3.00 for a brand-name drug and $1.50 for a generic. You cannot be denied a prescription because you cannot pay the copay.7Tennessee Secretary of State. Staying Approved: Renewals and Refill Rules
Initial approval is not permanent. At renewal, your prescriber has to document that you have lost at least 5% of your baseline body weight, are still participating in lifestyle changes, and are not combining the medication with any other weight loss drug. Since December 2025, extra refill controls apply to every GLP-1. You can only have one GLP-1 filled at a time, and refills are permitted only after 85% of the previous supply has been used. Bariatric Surgery as an Alternative
If injections are not the right path for you, TennCare covers bariatric surgery under a separate policy. Options include gastric bypass, sleeve gastrectomy, adjustable gastric banding (for members 18 and older), and biliopancreatic diversion. The BMI bar is higher: generally 40 or above, or 35 or above with a serious comorbidity such as poorly controlled hypertension, Type 2 diabetes, or obstructive sleep apnea. Could This Coverage Go Away
Coverage of obesity drugs is optional for state Medicaid programs under federal law, and Tennessee is one of a small group of states offering it. As of January 2026, only 13 state Medicaid programs covered GLP-1 drugs for obesity, down from 16 in 2025; California, New Hampshire, Pennsylvania, and South Carolina all dropped coverage during that period.9KFF.
