Utah does not have a state withholding form for employees equivalent to the federal W-4. Employers calculate Utah income tax withholding from the federal W-4 you already gave them, applying the state’s own withholding tables at Utah’s flat 4.5% rate. The form people usually mean when they ask about a “Utah withholding form” is the TC-40W, the Utah Withholding Tax Schedule, which you attach to your annual return to claim credit for state tax already withheld from your pay.
No Separate State W-4 for Employees
Many states make workers fill out a state-specific withholding certificate on top of the federal W-4. Utah skips that step. Your employer figures out how much Utah tax to pull from each paycheck using the filing status and adjustments on your federal W-4, then runs the numbers through the schedules in Utah Publication 14.1Utah State Tax Commission. Want more Utah tax taken out per paycheck? The cleanest way is to enter an extra dollar amount on line 4(c) of your federal W-4. That line tells your employer to withhold additional tax beyond the standard calculation, and because Utah rides on top of the W-4, the increase flows through to your state withholding too.3Internal Revenue Service. What the TC-40W Actually Does
The TC-40W is not a payroll form. You complete it at tax time to report every source of Utah income tax that was already withheld during the year. It works as a summary sheet: you transcribe the Utah-specific boxes from your W-2s and 1099s so the Utah State Tax Commission can match those amounts against what your employers and payers reported. The form has three parts, each for a different withholding source:
- Part 1 covers standard wage and income withholding reported on W-2s and 1099s.
- Part 2 covers mineral production withholding reported on Form TC-675R or a Utah Schedule K-1.
- Part 3 covers pass-through entity withholding reported on a Utah Schedule K-1, for partnerships and S corporations that withheld Utah tax on your behalf.

Who Needs to File It
Complete a TC-40W if any W-2, 1099, TC-675R, or Utah Schedule K-1 you received shows a Utah withholding amount. The form itself instructs you to enter W-2 or 1099 information “only if there is Utah withholding on the form.”4Utah State Tax Commission. TC-40W 2025 – Utah Withholding Tax Schedule If none of your income documents show any Utah tax withheld, you skip the TC-40W entirely.
Typical situations that produce Utah withholding include working for a Utah employer, receiving retirement distributions with Utah tax withheld, and earning income through a Utah-based partnership or S corporation that withheld on your share.
How to Fill Out Part 1
Gather every W-2 and 1099 that shows a Utah withholding amount before you start. You do not mail copies of those documents to the Tax Commission; the TC-40W replaces them. For each W-2 or 1099 in Part 1, enter:
- Line 1: the employer or payer’s federal ID number (W-2 box “b” or the payer’s EIN on a 1099).
- Line 2: the Utah withholding ID number (W-2 box 15 or on the 1099). This is a 14-character code ending in “WTH” with no hyphens.
- Line 3: the employer or payer’s name and address (W-2 box “c” or 1099).
- Line 4: an “X” if the entry comes from a 1099 rather than a W-2.
- Line 5: your Social Security number.
- Line 6: Utah wages or income (W-2 box 16 or the relevant 1099 amount).
- Line 7: Utah withholding tax (W-2 box 17 or 1099).
Once every W-2 and 1099 is entered, total the amounts from all line 7 entries. Where that total lands on your TC-40 depends on which parts you used. Submitting the TC-40W With Your Return
Attach the completed TC-40W to your Utah Individual Income Tax Return (Form TC-40). Don’t send it to your employer, and don’t submit it separately to the Tax Commission. It only travels as part of your annual return.4Utah State Tax Commission. TC-40W 2025 – Utah Withholding Tax Schedule
Utah’s TC-40 can be e-filed through the state’s Modernized Electronic Filing (MeF) program.5Utah State Tax Commission. Modernized Electronic Filing – MeF When you e-file, the TC-40W data transmits electronically with the rest of the return. Paper filers should print the TC-40W and include it in the envelope. Leaving it out can delay processing or cause the Tax Commission to reject your withholding credits.
