Does Washington State Insurance Cover Dental? Apple Health Plans

Washington state insurance does cover dental care, but the shape of that coverage depends on where you get your insurance. Apple Health, the state’s Medicaid program, includes dental benefits for both adults and children at no cost. Health plans sold through Washington Healthplanfinder do not include dental, so you buy a standalone dental plan alongside your medical coverage. Employers in Washington are not required to offer dental insurance to adult employees. And if you have no coverage at all, community health centers and a state referral service can connect you to care on a sliding fee scale.

Dental Coverage Under Apple Health

Apple Health, administered by the Health Care Authority, includes dental benefits across all its eligibility categories, including the Alternative Benefits Plan, Categorically Needy, Medically Needy, and Medical Care Services programs.¹ There are no copays and no deductibles for dental services.²

One point that trips up new enrollees: dental is not run through your Apple Health managed care plan. It’s administered separately through the state’s ProviderOne system, and you need to present your ProviderOne card at the dental office rather than your managed care plan card.³

What Adults Get

Adults 21 and older receive what the state calls comprehensive dental coverage, restored in January 2014 after the benefit had been cut back to emergency services from 2011 to 2014.² Covered services include routine exams, cleanings, X-rays, and fluoride; fillings and extractions; treatment for gum disease, with periodontal maintenance limited to four visits per 12 months for clients with diabetes; root canals on front teeth only;⁴ oral surgery; nitrous oxide for pain and anxiety;³ and dentures or partial dentures, with one initial set per client and replacement allowed once per lifetime after at least five years.⁵

Several services are excluded for adults. Apple Health does not pay for bridges, dental implants, porcelain crowns, or orthodontia for anyone 21 or older.⁶ Root canals on bicuspids and molars are not covered, and surgical periodontal procedures such as bone grafts and gum flap surgery are also excluded.⁷

Adults enrolled in Developmental Disabilities Community Services get expanded benefits, including topical fluoride three times per year, sealants on back teeth, and stainless steel crowns on posterior teeth with supporting documentation. Residents of skilled nursing facilities and alternate living facilities also receive certain services at increased frequency.⁶

What Children Get

Coverage for anyone 20 or younger is significantly broader, with no annual dollar cap.² Covered services include exams, cleanings, X-rays, fillings, sealants, fluoride, crowns, extractions, and prescriptions written by a dentist.⁸ Orthodontics is covered for children with qualifying conditions such as cleft palate, subject to prior authorization.³

Federal Early and Periodic Screening, Diagnosis, and Treatment rules go further: they require Apple Health to cover any medically necessary service for a child, even one that isn’t on the standard benefit list. Providers request the service through prior authorization, and the Health Care Authority reviews it case by case.⁹ Children six and under can also get no-cost care through the Access to Baby and Child Dentistry program.⁸

Who Qualifies for Apple Health

Eligibility uses household income measured against the Federal Poverty Level. Adults between 19 and 65 qualify at or below 138% of the FPL, roughly $21,597 per year for a single person as of April 2025. Children qualify for free coverage at or below 215% of the FPL, and low-cost premium coverage is available for households between 215% and 317% of the FPL.¹

Dental Plans on Washington Healthplanfinder

If you don’t qualify for Apple Health, dental coverage on the state’s ACA marketplace is sold separately from medical. Health plans on the exchange do not include embedded adult dental benefits, so you buy a standalone dental plan.¹⁰

For 2026, monthly premiums for a single adult run from $18 to $47, offered by Delta Dental of Washington, LifeWise WA, Dentegra Insurance Company, and Companion Life Insurance Company (EMI Health), with Kaiser Foundation Health Plan of the Northwest available in Clark and Cowlitz counties.¹¹ You can enroll during the open enrollment window from November 1 to January 15, or during a special enrollment period after a qualifying life event.¹² Premium tax credits do not apply to standalone dental plans.¹⁰

Pediatric Dental Is Required

Pediatric dental care is an essential health benefit under the ACA for children under 19. Washington meets that requirement through the standalone dental plans on the exchange rather than by folding dental into health plans.¹³ Any child 18 or younger enrolled in a qualified health plan through the exchange must also be enrolled in a dental plan.¹⁰ Out-of-pocket costs for pediatric dental on these plans are capped at $450 per child or $900 per family in 2026, with no cap on medically necessary benefits.¹²

What Employers Have to Offer

Washington state law does not require employers to offer dental insurance. Under the ACA, employers with 50 or more full-time employees must provide minimum health benefits or face a penalty, but there is no federal mandate for separate dental or vision coverage for adults. The one exception involves employees’ children: if an employee has a child under 19, the health plan must cover essential health benefits including pediatric dental and vision.¹⁴

Employers with fewer than 50 full-time employees have no ACA obligation to provide any health insurance. Washington law lets local government entities such as counties and municipalities provide health benefits to employees, but that authority is permissive, not mandatory.¹⁴

If You Have No Coverage

DentistLink is a free nonprofit referral service, run by the Arcora Foundation in partnership with the Health Care Authority, that connects both insured and uninsured residents with providers. You can search the directory at dentistlink.org, call or text a referral specialist at 844-888-5465 on weekdays, or submit a request form. Interpretation in multiple languages and help arranging transportation are included.¹⁵

Federally Qualified Health Centers around the state offer dental care on a sliding fee scale based on household income and family size. HealthPoint, serving King County, offers appointments starting at $50 and says no one will be denied care for inability to pay.¹⁶ Community Health Care in Pierce County provides preventive and restorative dental services and accepts Apple Health, most private insurance, and sliding-scale payment from uninsured patients.¹⁷ The Community Health Center of Snohomish County runs a similar discount program based on documented household income.¹⁸ The 211 helpline (dial 211 or visit win211.org) and the Washington Healthcare Access Alliance search tool also maintain lists of free and reduced-cost clinics.¹⁹

What’s Changing in 2026

The Health Care Authority set up the Apple Health Oral Health Advisory Workgroup in January 2025 to recommend benefit improvements, focused on expanding access for people with disabilities and evaluating new service codes. No new codes have been finalized yet. On the administrative side, crown build-up and post-and-core services no longer require prior authorization for clients 20 and younger, and new prior authorization forms for dental prostheses took effect in April 2026.²⁰

Budget proposals from the 2025 legislative session included cuts to Apple Health dental reimbursement. The House budget package proposed reducing adult dental rates by roughly $38 million and children’s dental rates by roughly $46 million over the 2025–2027 biennium.²¹ Dental advocates have said the reductions could shrink the number of providers willing to see Apple Health patients.
