Driving Records and Crash Reports

To request a copy of your driving history for an employer, insurer, or court, call the driver license record request line at 850-617-2573. For crash reports, call 850-617-3416 and select Option 5.² Driving records are also available through your local county court clerk.³

Medical Review

If you’ve been notified of a medical review and haven’t heard back within 30 days, call the Medical Review Board at 850-617-3814.

License Suspensions and Reinstatement

Formal review hearings are handled by the Bureau of Administrative Reviews, which you can reach through the general customer service number, 850-617-2000. Reinstatement fees vary by the reason for the suspension, and they must be paid in full before driving privileges are restored.⁴

What to Have Ready Before You Call

Agents need specific identifiers to open your record, and being asked to call back is a common outcome when you don’t have them.

Your Florida driver license number, printed on the front of the card.
The last four digits of your Social Security number, used for identity verification.
Your Vehicle Identification Number for any title, registration, or tag question. The 17-character VIN is visible through the windshield on the driver’s side of the dashboard, or on a sticker inside the driver’s-side door jamb.

For a suspension or reinstatement call, pull any citation numbers, case numbers, and court paperwork before you dial so you can confirm the agent is looking at the right record.

When You Don’t Need to Call at All

A lot of the reasons people dial FLHSMV can be handled faster online or at a local counter.

GoRenew

The GoRenew portal at services.flhsmv.gov/virtualoffice handles license and ID renewals and replacements, vehicle and vessel registration renewals and replacements, and paper title requests. A non-refundable $2.00 convenience fee applies on top of the standard state fee. Before you pay, run a free Driver License Check or Motor Vehicle Check through the same portal to catch any hold that would block the transaction.⁵

Email Forms

For non-urgent questions, FLHSMV maintains web forms for driver license and ID issues and a separate one for motor vehicle tags, titles, and registration. Both are linked from the Contact Us page. Responses are not same-day during busy periods.¹

County Tax Collector Offices

For routine walk-in business, your local county tax collector is usually faster than the state line. Tax collectors act as authorized agents of FLHSMV and can collect fees and update state records directly.⁶ Not every office offers every service, though, so call first. Driving tests and CDL services in particular are available at some locations but not others.⁷ Phone numbers, addresses, and hours for each county are at flhsmv.gov/locations.⁸
