What the Claim Form Asked For
If you want to confirm your filing was complete, here is what a valid submission required. The form used the Unique ID printed on the notice letter mailed or emailed to affected patients. That ID pre-populated your name and contact details from the administrator’s records.
You then selected one or both benefit categories: reimbursement for documented out-of-pocket losses, the pro rata cash payment, or both. For out-of-pocket losses, you entered a dollar amount for each expense and uploaded supporting documents such as receipts, bank statements, or invoices. What an Approved Claim Pays
The settlement provides three forms of relief from an $11.5 million fund.
Reimbursement of up to $5,000 covers documented out-of-pocket losses traceable to the breach. Eligible expenses include unreimbursed fraud or identity theft losses, credit monitoring fees, postage, copying, mileage, parking, notary charges, usage-billed phone and data charges, bank fees, accountant fees, and attorney fees. A pro rata cash payment of up to $599 goes to every class member who filed a valid claim, whether or not they also claimed out-of-pocket losses.5BankInfoSecurity. If You Already Filed a Claim
The court held the final fairness hearing on May 20, 2025, and granted final approval. If You Missed the Deadline
New cash claims are no longer accepted. The one benefit still on the table for class members who did not file is enrollment in two years of Medical Shield Complete. If you have not yet enrolled and are still within the enrollment window, check availability at the settlement website. This benefit was extended to every class member, not only to people who submitted a claim form.
Taxes on Your Payment
Data breach settlement payments are generally taxable income. The IRS excludes damages from gross income only when they compensate for physical injuries or physical sickness, and payments for data exposure, identity theft losses, and lost privacy do not meet that standard.7IRS. Starting in 2026, the IRS reporting threshold for settlement payments on Form 1099-MISC rises from $600 to $2,000 per payee per calendar year. If your total payment falls below $2,000, the administrator may not issue a 1099-MISC, but the income is still reportable. Keep a record of the amount you receive for your return.
