How to Complete and Submit the Norton Data Incident Settlement Claim Form

To claim money from the Norton Healthcare data breach settlement, submit the Norton data incident settlement claim form online at nortondataincidentsettlement.com or by mail to Kroll Settlement Administration, postmarked no later than May 18, 2026.1Norton Healthcare Data Incident Settlement. Norton Healthcare Data Incident Settlement The $11 million fund covers roughly 2.5 million people notified that their information may have been exposed in the May 2023 incident, and the court is scheduled to decide final approval on May 15, 2026.

Confirm You’re Eligible Before You File

You qualify if you received a notification letter from Norton Healthcare about the May 2023 data incident.1Norton Healthcare Data Incident Settlement. Norton Healthcare Data Incident Settlement That letter is your proof of class membership, and it carries the class member ID that links your claim to Norton’s records. If you think you should have been notified but weren’t, or if you’ve lost the letter, the settlement website lets you look up your eligibility, and the settlement administrator can send a replacement.

Decide Which Benefits to Claim

The form lets you select from four benefit categories, and you can claim more than one. Two require documentation; two do not.2Norton Healthcare Data Incident Settlement. Frequently Asked Questions

  • Out-of-pocket losses up to $2,500, for unreimbursed expenses fairly traceable to the breach. Documentation required.
  • Lost time at $20 per hour for up to four hours, capped at $80. A written description is required.
  • Three years of CyEx Medical Shield Pro medical monitoring, delivered through an enrollment code. No documentation required.
  • A cash payment of at least $5, adjusted up or down depending on how many valid claims come in. No documentation required.

Gather Your Documents First

Before opening the form, pull together your notification letter and anything that supports a dollar claim.

For out-of-pocket losses, that means receipts, invoices, or statements showing what you paid and when. Common reimbursable expenses include credit monitoring or identity theft protection you bought after the breach, bank fees for closing and reopening accounts, charges tied to credit reports and credit freezes, and unreimbursed fraudulent transactions traceable to the incident. A credit monitoring subscription purchased in June 2023 after you received Norton’s letter is a clean claim. A vague charge from two years later with no paper trail connecting it to the breach will likely be denied. Bank or credit card statements showing fraudulent activity help when you’re claiming reimbursement for unauthorized charges.

For lost time, keep a short written log of what you did and roughly how long it took. “Spent 45 minutes on the phone with my bank on June 12, 2023, to freeze my debit card” is far more convincing than “spent time dealing with the breach.”2Norton Healthcare Data Incident Settlement. Frequently Asked Questions Phone calls to financial institutions, setting up fraud alerts, reviewing credit reports, and filing police reports all qualify.

Fill Out the Form

The form asks for your full legal name, current mailing address, and email. Enter the class member ID from your notification letter in the field provided; this is the single most important entry on the form because it ties your submission to Norton’s records and speeds verification.

Check the box for each benefit you want to claim. If you’re claiming out-of-pocket losses, itemize them and attach your receipts. If you’re claiming lost time, write out what you did, when, and how long each task took. For medical monitoring and the cash payment, you only need to confirm your class membership and check the box.

Submit Online or by Mail

The fastest route is the online portal at nortondataincidentsettlement.com. It walks you through each section, lets you upload supporting documents as PDFs or image files, and generates a confirmation when you’re done. Save or print that confirmation as proof of timely filing.

For paper filing, download and print the form from the settlement website, or call the administrator to have one mailed to you. Attach copies of your supporting documents, keep the originals, and send everything to:

Use certified mail or another method that produces a postmark receipt. Online submissions and mailed forms both must be in by May 18, 2026.1Norton Healthcare Data Incident Settlement. Norton Healthcare Data Incident Settlement A claim postmarked May 19 is a claim denied.

What Happens After You File

The administrator reviews each claim for completeness. If something is missing or unclear, you’ll receive a deficiency notice with a short window to supply the missing receipt, clarify a date, or add detail. Respond quickly, because letting the window lapse can kill an otherwise valid claim.

The court is scheduled to hold a Final Approval Hearing on May 15, 2026. As of this writing, the settlement is not yet finally approved. If the judge approves it and no appeals are filed, payments go out after the appeals period expires, typically several months later. An appeal freezes the entire payout until it’s resolved, which can add a year or more. Payment goes to the address or account information on your claim form, so update your details with Kroll if you move.

Opting Out or Objecting Has an Earlier Deadline

If you’d rather preserve your right to sue Norton Healthcare independently, you have to opt out. Accepting any settlement benefit waives claims related to the breach, including negligence, breach of implied contract, unjust enrichment, and invasion of privacy. The opt-out and objection deadlines are both April 20, 2026, almost a month before the claim deadline.3ClassAction.org. Notice of Proposed Class Action Settlement – Abby Berthold, et al. v. Norton Healthcare, Inc., et al.

To opt out, mail a signed written request with your full name, current address, the case name (Abby Berthold, et al. v. Norton Healthcare, Inc., et al.), and a clear statement that you want to be excluded, postmarked by April 20, 2026, to the Kroll address above. Objecting is different: you stay in the class but tell the court you think the terms are unfair. Objections must be filed with the Jefferson Circuit Court and copied to class counsel, defense counsel, and the administrator by the same date. The long-form notice on the settlement website lists each address and the required contents.

Expect Tax Paperwork if Your Payout Is Large

Payments that reimburse actual out-of-pocket losses you already incurred, such as bank fees or credit monitoring costs, generally aren’t taxable because they restore you to your pre-loss position rather than generating new income. The flat cash payment may be treated as taxable income because it isn’t tied to a specific documented loss. Payers must report settlement payments of $600 or more on a Form 1099, so if your total crosses that threshold, expect tax paperwork the following January. Keep the receipts you submitted; they’re your defense if the IRS questions whether a payment was a nontaxable reimbursement. For amounts near or above $600, a tax professional can tell you what you need to report.
