What the Form Asks For
You can download the form from www.mountsinaisettlement.com or fill it in directly on the site. It has five short sections:4ClassAction.org. Cooper v. Mount Sinai Health System, Inc. Claim Form
- Contact information: name, mailing address, email, and an optional phone number.
- Eligibility certification: check the box confirming you logged into MyChart during the class period, and enter the Unique ID Number from your notice.
