How to Fill Out and Submit the Numotion Settlement Claim Form

To file a claim in the Numotion data breach settlement, complete the Numotion settlement claim form online at numotionsettlement.com or print it and mail it to Kroll Settlement Administration so it’s postmarked by March 18, 2026. Class members can claim up to $15,000 in documented out-of-pocket losses, a share of any money left in the $4 million fund, and — if their Social Security number was involved — two years of medical account monitoring. Two years of credit monitoring goes to every class member automatically, whether you file or not.

Who Can File

You’re in the Settlement Class if you’re a living person in the United States who received a notice from Numotion about either the February–March 2024 incident or the September 2024 phishing incident. Receiving that notice is the qualifier. You don’t have to show your information was actually misused.

A smaller Social Security Class covers people whose Social Security numbers were among the data involved. Members of that group can claim an extra medical monitoring benefit on top of everything else.

Every class member automatically receives two years of CyEx Identity Defense Plus credit monitoring at no cost. No claim form is needed for that piece; enrollment instructions come from the settlement administrator.

If You Can’t Find Your Notice

The mailed or emailed notice contains a Settlement Class Member ID that the claim form requires. If yours is lost, call Kroll at (833) 630-5361 or use the contact form at numotionsettlement.com. What You Can Claim

Documented Out-of-Pocket Losses (Up to $15,000)

This is the largest individual benefit. You can request reimbursement for money you spent or lost because of the breach: credit monitoring you paid for on your own, bank fees from unauthorized transactions, identity theft recovery costs, and similar expenses. Each item must be “fairly traceable” to the March 2024 or September 2024 incident, and every dollar has to be backed by documentation — receipts, invoices, bank statements, or similar records.

Lost time counts too. Hours spent freezing accounts, filing fraud reports, disputing charges, or monitoring your credit are compensable. Describe what you did and how long it took. Concrete beats vague: “three hours on the phone with Chase fraud department on April 15, 2024” is far more useful than “dealt with breach issues.”

Pro Rata Cash

Anything left in the $4 million fund after documented-loss claims, fees, and administrative costs is split among class members who filed valid claims. You have to submit the form to be included, but no proof of loss is required for this portion. The per-person amount depends on how many people file and how much the documented-loss claims consume first.

Medical Monitoring (Social Security Class Only)

If your Social Security number was involved, you can claim two years of CyEx Medical Shield Pro medical account monitoring. This is separate from the automatic credit monitoring. A working email address on your claim form is required to receive it.

Filling Out the Form

Whether you file online or on paper, you’ll need the same core information: your Settlement Class Member ID from the notice, your full legal name, current mailing address, and email address. The email matters especially if you’re claiming medical monitoring or want electronic status updates.

For documented losses, list each expense on its own line with a description, the date, and the amount, and attach supporting documents for every entry. The administrator won’t approve expenses without proof. For lost time, spell out the activities and estimate hours for each.

For pro rata cash, check the box indicating you want to be included. Nothing else is required for that category.

For medical monitoring, check the box (it only appears if you’re in the Social Security Class) and confirm your email.

How to Submit

Same deadline applies both ways: March 18, 2026.

Online

Go to numotionsettlement.com, enter your Settlement Class Member ID, fill in your details, select your benefits, and upload documents. After you submit, save the confirmation number the site generates.

By Mail

Print the form from the settlement website, fill it out clearly, and mail it with copies of your supporting documents to:

The envelope must be postmarked by March 18, 2026. Illegible handwriting can delay or defeat a claim. Keep copies of everything.

Key Deadlines

  • March 3, 2026 — deadline to opt out or file an objection (postmark).
  • March 18, 2026 — deadline to submit or postmark your claim form. Miss it and you get no cash payment, though the automatic credit monitoring still applies.
  • April 2, 2026, 2:30 p.m. CT — Final Approval Hearing.

Opting Out or Objecting

Staying in the settlement means giving up the right to sue Numotion separately over these breaches.

Opting out is the only way to preserve your own lawsuit rights. If you opt out, you get nothing from the settlement: no cash, no credit monitoring, no medical monitoring. Your written request must be postmarked by March 3, 2026.

Objecting is different. You can file a written objection with the court by March 3, 2026, explaining why you think the terms are unfair, and still remain in the settlement and collect benefits if the court approves it. You can also ask to speak at the April 2 hearing.

After You Submit

Kroll reviews each claim against the official list of affected individuals. If documentation is missing or amounts look inconsistent, the administrator may email you asking for more. Respond quickly. Ignoring a request can reduce or kill your claim.

No payments go out until after the court grants final approval on April 2, 2026. An appeal, if one is filed, pushes disbursement back until it resolves. Otherwise, checks or electronic payments generally arrive within a few months of final approval.

Taxes on What You Receive

The IRS treats settlement payments as taxable income unless a specific exemption applies. Reimbursement for money you actually spent may not create taxable income because it only makes you whole, but the IRS looks at what each payment replaces. Pro rata cash that isn’t tied to a specific documented loss is more likely to be taxable. There’s no blanket exemption for data breach settlements, so different parts of your payment can be treated differently. For a significant payment, talk to a tax professional before you file your return.
