Wilfredo Tortolero Arriechi Case: Charges, Detainer, Fallout

Wilfredo Jose Tortolero Arriechi is a 34-year-old Venezuelan national charged with murdering Alberto Rangel, a social worker who was fatally stabbed on December 4, 2025, at the Ward 86 HIV clinic inside Zuckerberg San Francisco General Hospital. A San Francisco Superior Court judge ordered him to stand trial for murder on June 29, 2026, and he remains in custody at San Francisco County Jail without bail.¹

What Happened at Ward 86

Tortolero Arriechi was a patient at the outpatient HIV clinic at San Francisco General. In the weeks before the attack, staff logged a series of alarming visits. On November 13, 2025, he came looking for his physician, who was absent, and was escorted out. Around November 20, he met with his doctor and threatened to sue him. Staff reported the “elevated” behavior to the Department of Public Health’s security team, which tried to phone him between November 24 and 26 but never reached him.²

On the morning of December 4, he returned again asking for his doctor, who was again absent, and told staff he would be back at 1:00 p.m. Clinic staff alerted DPH security and a sheriff’s department lieutenant. A background check turned up no outstanding warrants, and a deputy was dispatched to guard the doctor.²

Tortolero Arriechi arrived around 1:30 p.m. and was connected with Rangel. Witnesses described the initial conversation as calm. At approximately 1:39 p.m., as the two walked toward an elevator, he grabbed Rangel from behind and stabbed him multiple times in the neck and shoulder with a concealed five-inch kitchen knife.³ A colleague, clinical social worker Alejandro Alvarez, pulled the attacker off Rangel by the back of his shirt, causing the knife to drop.⁴ A nearby sheriff’s deputy then restrained and detained him.

Rangel was hospitalized and died two days later, on December 6, 2025.³

Who Alberto Rangel Was

Rangel was 51 and worked as a social worker for the University of California, San Francisco. At Ward 86, he ran HIV support groups in both English and Spanish. Colleagues said he “treated everyone with love and absolute care” and “went above and beyond for his colleagues and patients.” Outside work, he was remembered as an artist and athlete with a sharp sense of humor and style.⁵ A memorial was held outside the hospital on December 9, 2025.⁶

Rangel’s husband, Stuart Moulder, has filed a wrongful death claim against the city of San Francisco, alleging the tragedy was “entirely foreseeable” and that measures such as single-point entry and weapons screening would have prevented it.⁷

Background of the Suspect

Tortolero Arriechi entered the United States illegally in 2023. He was encountered by U.S. Border Patrol that year and released into the country.⁸ His social media accounts, reviewed by investigators and journalists, documented a journey from Venezuela through Colombia to San Francisco marked by attempts to find work and survive economic hardship.⁹

In the months before the stabbing, his posts reflected an apparent mental health decline. In October 2025, he posted an Instagram Threads photo showing his Ward 86 visit summary with a knife stabbed through the document into a doorframe, captioned: “The knife is to kill the door. It’s always open.” An October 28 video showed him rehearsing a conversation with his doctor about medication. On November 28, less than a week before the attack, he wrote: “They are killing me, they are grabbing me. I’m not going to hand over the knife so they can kill me.” In another post that same day he wrote: “I also want to confess to you that I have been thinking a lot on the topic of death, killing and dying, the responsibility of giving or taking a life.” On December 3, the day before the stabbing, he posted a video saying: “You know, so they can see it after my death. Because that’s how we artists are.”⁹

The Criminal Case

San Francisco District Attorney Brooke Jenkins charged Tortolero Arriechi on December 8, 2025, with one count of murder, with an allegation of personal use of a deadly weapon. The charges had initially been filed as attempted murder and were upgraded after Rangel died. The DA’s office sought detention without bail, citing public safety.¹⁰

Immediately after his arrest, he was placed on a psychiatric hold that lasted weeks, and his arraignment was repeatedly postponed while he remained under evaluation.³ He appeared in court on January 7, 2026, and pleaded not guilty. Public defender Sylvia Nguyen said he had been suffering from serious mental conditions and was in a “psychotic state” at the time of the attack, and that he had needed roughly three weeks to stabilize before he could attend court.¹¹

Assistant District Attorney Edward Mario is prosecuting the case. At a preliminary hearing on June 29, 2026, that lasted less than two hours and included testimony from the city’s chief medical examiner and two sheriff’s deputies, Superior Court Judge Michael Rhoads ordered him to stand trial for murder. As of that hearing, no formal insanity defense had been entered, though the defense had publicly emphasized his mental health. His next court hearing was scheduled for July 13, 2026.¹

Immigration Detainer and Federal Response

On December 11, 2025, ICE lodged an immigration detainer against Tortolero Arriechi, requesting that San Francisco hold him for federal authorities after any criminal sentence is served.⁸

ICE Director Todd M. Lyons said Tortolero Arriechi “illegally entered the U.S. in 2023, was let go under the Biden administration’s catch-and-release policies, and ended up in the sanctuary city where Gavin Newsom was once mayor and Kamala Harris was the district attorney.”⁸ In a May 2026 statement, DHS Acting Assistant Secretary for Public Affairs Lauren Bis said: “If it weren’t for the Biden administration’s reckless open-border policies, Alberto Rangel would still be alive. We are calling on Governor Gavin Newsom to commit to not releasing this murderer from jail. California’s sanctuary policies put American lives at risk.”¹²

In February 2026, Lyons sent a letter to California Attorney General Rob Bonta calling on the state to honor ICE arrest detainers for more than 33,000 undocumented immigrants then in California custody, citing this case as part of a broader pressure campaign against sanctuary jurisdictions.¹²

Institutional Fallout

The killing exposed security failures at Ward 86 that both an internal Department of Public Health audit and state workplace safety inspectors later documented. A 13-page DPH assessment released in April 2026 found a “lack of adequate safety protections” at the clinic, noted that no clinic-wide safety plan was developed after staff reported the threats, and confirmed that a coworker rather than the deputy first physically intervened.¹³

In June 2026, Cal/OSHA cited San Francisco General Hospital with seven violations, six classified as serious, and proposed a $130,500 fine. UCSF received eight citations, seven serious, with a $142,700 proposed fine, for failures that included lacking an effective workplace violence prevention plan and not immediately reporting the stabbing to Cal/OSHA.¹⁴ San Francisco subsequently committed nearly $23 million to security improvements across its public health facilities, including weapons detection, a 24/7 threat management team, and new training and staffing.¹⁵
